Sanity is a new resource being introduced in patch 8.3 Visions of N'zoth. The resource works as a timer for how long you can stay inside a Lesser Vision or Horrific Vision, as your attempt is over when your Sanity reaches 0.
Sanity
is a player resource bar that starts at 1000 by default. This is drained passively over time and drained in larger amounts if you are hit by certain abilities.
If your Sanity hits 0, you die and are forced out of the Vision.
The reverse is also true; if you die, you'll be reduced to 0 Sanity.
If you are in a group, you won't be forced out of a Vision until all party members are dead, as there are ways for alive players to revive dead players.
Losing Sanity in Horrific Visions of N'zoth
You lose Sanity due to all of the following while in Visions of N'zoth:
Passive Sanity Drain
- While in any Vision of N'zoth, your Sanity is passively drained just by being inside the vision. In Horrific Visions, the passive Sanity drain varies depending on which area of the Vision you are standing in.
Lesser Vision of N'zoth: -5 Sanity per sec
Horrific Vision Tainted Area: -6 Sanity per sec
Horrific Vision Corrupted Area: -8 Sanity per sec
Horrific Vision Lost Area: -12 Sanity per sec
Sanity Damage from Creatures
- Some enemies inside Visions of N'zoth cast abilities that drain your Sanity if you are afflicted by them. In Horrific Visions, any enemy that has an ability that can drain your Sanity will have a buff on it called:
Drinking a -Sanity Potion Vial
- Hidden around Horrific Visions, you may find colored potions. There are 5 colors that have effects based on their color. One color, however, will drain you of some of your Sanity.
Death
- If your health hits 0, you will also be brought down to 0 Sanity.
Void Zones
- If you activate this Faceless Mask, you will spawn void zones behind you as you move. Standing in those void zones will drain your Sanity.
Reviving Another Player
- If another player in your party dies while in a Vision of N'zoth, you can give half of your remaining Sanity to revive them. The amount of Sanity lost by reviving another player ignores any Sanity resistance effects.
Sanity Loss Resistance in Horrific Visions of N'zoth
All of the above mechanics that cause you to lose Sanity can be mitigated by the power of your

reduces all Sanity draining effects while in the Visions of N'zoth.
is a Titanic Research upgrade that has a chance to proc when defeating enemies in Horrific Visions. When this effect occurs, for 20 seconds you'll be immune to all Sanity loss effects.
Gaining Sanity in Horrific Visions of N'zoth
All of these below effects either restore or increase your Sanity capacity.
is the first Titanic Research upgrade players will have. When purchased, your party will have access to 3 Sanity Restoration Orbs that you can deploy by clicking the extra action button on your screen while inside a Horrific Vision. This will fully restore your Sanity over its duration as well as revive any dead players with 100% Sanity.
is a Titanic Reserach upgrade. When purchased, killing Elite and Boss enemies inside Horrific Visions will restore 200 Sanity.
- Another Titanic Research upgrade. If you die or lose all of your Sanity, you'll be instantly revived with 500 Sanity and full health. You can only benefit from this once per Horrific Vision.
- Another Titanic Research upgrade. This increases you and your party's maximum Sanity by +50 per rank. There are 3 ranks. You will also benefit from other players in your party who have purchased this upgrade, causing their effects to add up.
Drinking A +Sanity Potion Vial
- Hidden around Horrific Visions, you may find colored potions on the ground. There are 5 colors that have effects based on their color when you interact with them. One of these colors will restore 100 Sanity instantly when used.
Being Revived By Another Player
- When you die, your Sanity will be set to 0. Another player in your party can revive you by donating half of their remaining Sanity. You will be revived with the amount of Sanity a party member gave up. If the player reviving you has
available, you'll be revived at 100% Sanity.

. It can be used in any Visions of N'zoth on a 3-minute cooldown. It restores 100 Sanity if you spend 10 seconds eating it.

It's not mentioned here, but the Sanity Restoration Orb CANNOT be cast while in combat, so make sure you use it while out of combat. I used it just before a pull so that the over-restoration was alleviated some.

I haven't noticed the vials, any idea where they might be hidden or can someone share a photo? Also I read somewhere that a totem spawns once you complete a tainted or lost area that when you click on it teleports you outside thralls room still haven't seen any. I have done only tainted areas though could it be only after you clear a lost area?

I figured out how to tell which Vials damage your sanity in visions, so no more worrying about clicking the wrong one!
At the Coords 47.05, 81.01 there is a small hut (next to the blacksmiths) inside this hut there is a dead alchemist with a note, clicking the note will reveal the text ends in "do not drink the.." and he's killed before he can finish the note
However next to him lies a dead Voidbound Ravager, I assume that's the guy that whacked our poor alchemist, it would seem after doing that he attempted to drink a vial of liquid which may have killed him.
Whatever colour the Vial is in his/her arms, is the colour vial that will damage your sanity in the vision, so just avoid drinking that colour you find in the hut for the rest of the vision.

K'Bab is a food item obtained through Cooking. It can be used in any Visions of N'zoth on a 3-minute cooldown. It restores 100 Sanity if you spend 10 seconds eating it. This effect can occur twice per use, as the total eating time is 20 seconds. So when you use these, make sure to sit the full 20 seconds for 2 ticks of +100 Sanity.
Not sure what other's experience has been, but I have tried multiple times and have never received a 2nd tick of the +100 Sanity. I have taken it to the full 20 second duration on multiple occasions in both SW & Org, and found that I essentially wasted 10 seconds worth of sanity in doing so. Just as an FYI... as not sure if this is how was intended, was a bug, or was a nerf at some point? Also not sure if it is based on the the timer triggering at 10.001 seconds to give you the +100, leaving you with only 9.999 remaining time in the duration, therefore nullifying the ability to get that 2nd tick. Again, beware as sitting for the additional 10 seconds can almost negate the benefit depending on the zone where you consume.
