This item could be called one of the best caster trinkets for a majority of T6 content, including all of Hyjal, Teron Gorefiend, Supremus, Mother Shahraz, and Illidan Stormrage.
It is also useful for select bosses in lesser content, including Leotheras the Blind, Magtheridon, Netherspite, Terestian Illhoof, Attumen the Huntsman, Moroes, Shade of Aran, and Prince Malchezaar.
Yeah, against all those listed bosses it's arguably best trinket in the entire game. Comparabe to Skull of Gul'dan, better than Darkmoon Card: Crusade...
Eventhough the trinket from Al'ar is rather great for Tankadins, I'm trying to get my guild to take a run through Naxxramas for this one. It's quite a lot of spell damage, and very useful in places like Hyjal where you need to get a lot of threat on a lot of much as fast as possible.
EDIT: It's probably quite good at Supremus as well. I'm currently the Main-tank on him, due to my lower health points. And it can get quite bothering if one of the tanks starts attacking him before I can do so, or if the dps are doing it. Of course I can get aggro in one Exorcism if the trinket has procced, but it usually procs on the Judgement twenty seconds or so before he changes phase again.
Great threat trinket for protection paladins against demon and undead.
100% drop chance from Kel'thuzad. Consider grabbing this one.

When all the other boss head turn-ins grant extremely useful and powerful epic items, the most powerful boss so far drops a Phylactery which can be turned in for a situational trinket. Woot.
These also add +85 damage to DEMONS! That makes it a good deal better. I'm sure it will be more useful in the expansion. Demons and Undead are a wide variety of enemies. Still, kinda sucks.
This would be useful for the hunter epic quest, although if you get this you probably have something much better than Rhok.
Perhaps there are enough demons in the Outland to make this worth equipping?
i'm just wondering what you'd need the scourge stones for at that level.
In case anyone mysteriously failed to notice, Blizzard will be releasing an expansion pack soon. This expansion pack takes players to the world of Outland, where the agents of the Burning Legion seek to rebuild their strength for a new assault on Azeroth.
That means demons, and lots of them. Enough demons to make your head spin. If you were even slightly more aware than an amoeba, you'd figure out that these trinkets will be awesomely powerful for battling these creatures.

I've taken the liberty of rewriting/attempting to rewrite that big ass block of text you see above. Sorry to whatever errors I make...I've only played 6 lvls into WCIII RoC soooo...bear with me :D
By the way anything in It is hard as hell, as you have 3 armies and you have to use your Nightelves to defend the Worldtree for 45 mins.
You're also fighting agents of the Burning Legion and it is a hardass fight. You'll probably lose 2 of 3 bases before you win. It's also easy to lose your final base if you haven't built up defenses right throughout the level. It basically becomes pointless to continue without cheats.
If Kel'thuzad is showing problems to people now, imagine the difficulty when they bring in Kil'jaedin, Illidan's former master and the keeper of the Dreadlords (OOOOOOOOO scary )
Kil'jaedin will probably be 3 to 4 times harder than Kel'thuzad. Warriors, I'd suggest having a demonslayer or some sort of anti-demon weapon as Kil'jaedin will be the hardest battle you've faced as of yet. And, healers probably won't be able to keep you alive for long unless they are properly equipped. (Time to start become holy priests.)

As you can see I have a HELL of a lot of time on my hands. Although, this guy has a point, demons are the future of WoW, oh and, so far we still have yet to need shadow and arcane resist gear...can't wait to farm that, no more room in my bank >_< (as if there was any room now!)

Undead and DEMONS! A BC trinket maybe?
This is a passive situational trinket, and it rocks the freaking house.
+85 spl dmg without having to hit the 'use' is awesome. This is good for Kara, Mag, & Mt Hyjal (perhaps certain bosses in BT, but I don't know).
Shake N Bake...
This is the KT "head" caster reward trinket. kinda sad considering hes the hardest boss there is, and this only works on undead and demons, but 85 dmg is a lot : /

Best in slot for Naxx, but how long does it last into BC?
Simply put, there aren't many trinkets that give an equivalent of 85 spellpower, and for most caster specs it outsims all trinkets until The Skull of Gul'dan. You can expect to be using this for quite some time into BC Classic.
