Shadow Word:Pain ticks 8 times with Improved talent, so that's 8 chances for this to proc. The proc duration is 10 seconds.
Renew ticks 5 times, 5 chances to proc. The proc duration is 5 seconds.
I find it interesting that the proc grants an equal amount of spell damage or healing power. This trinket is clearly weighted towards SW:P

Can you get the buff twice? Say a SW:P proc and a Renew proc?
That would be essentially 400 healing, which would throw it back into renew's favor if you keep up a rank 1 SW:P on bosses and such.

This may seem weighted towards shadow priests, but remember that this can proc off of multiple casts of the spell.
In a raid, if you aren't the main healer, you might have renew on 5+ targets, meaning you'll pretty much have the buff 100% of the time.
You're more likely to have SW:P on multiple targets in PvP over PvE, and even then probably not too many, so the two buffs balance out.

Renew actually has to heal in order to proc the trinket, so you can't just throw renew on everyone and assume the trinket will always provide the buff. It's definatly a shadow priest trinket over a holy priest

Ashtogue Vs. Passive Damage Trinkets (Single Mob)
This trinket on average will proc' twice a minute for a total of 20 seconds (assuming it doesn't proc' over its self etc). That means 220 damage for 20 seconds, or roughly an average of 73.33 +damage for the entire minute.
Of course this number assumes SW:Pain ticks exactly every 3 seconds, and that it never proc's over its self. If we take both into account, we can estimate roughly 62 +Damage on average.
This rough number means it is the second best +Damage trinket for a Shadow Priest in WoW, the first being the Darkmoon: Crusade card with its +80 damage passive effect.
These numbers also assume the Priest only has a single Shadow Word: Pain active on a single enemy which is often, but not always the case. In any scenario where SW:Pain is active on more than one enemy, this trinket might offer, on average, more +Damage than even the Darkmoon: Crusade card.

I did some testing with this over a night of attempts on RoS.

It's a little hard to read, but I'll summarize:
I had the talisman on all night, and I had a grand total of 2446 Renew ticks. Now, if you scroll down to the Buffs and Debuffs section, you see that I gained the buff, Divine Blessing, 150 times. This would lead me to believe that i got 150 procs for 2446 Renew ticks, or roughly a 6.13% proc rate. Much lower than the advertised rate.
I also did a bit of Shadow Word: Pain as well, but the data sample is much smaller, at 139 ticks. I gained the damage buff, Divine Wrath, 7 times, or roughly a 5% proc rate. Granted the SW: P sample is much lower, so I wouldn't base much on it, but the Renew sample is pretty good.
Based on my testing, I'd bet that there is a hidden cooldown on the trinket that doesn't allow it to proc as often as advertised. The other explaination is that it can't proc if you already have the buff on you, meaning you'd have to allow it to expire before it would be reapplied. Bottom line, as a healer I'd prefer to use a different trinket than this, based on the fact that 1) I don't massivly spam Renew, and most of the time my Renews are flashed over, meaning they don't get a chance to tick in most casts, and 2) it provides no passive effects that would give me more mana regen. It's an interesting trinket for healers, but it doesn't seem to be great if it has a hidden cooldown.
As far as the damage side of it goes, I can't really get a good sample of data as I am not a shadow priest, but if the hidden cooldown exists, I don't think it would be much better for a shadow priest.

There are a few misconceptions in the above comments
This trinket has no cooldown on its proc, so it is very possible to have itself refresh midway through the proc. As such, you will not see yourself gaining the buff for a second time since you already have it. So Eiras test percentages are likely to be incorrect.
For single target pain, it is easiest to calculate in 3 second intervals after pain has been cast
1st three - 0%
2nd three - 10% (1st tick proc)
3rd three - 19% (1st or 2nd tick proc)
4th three - 27.1% (1st, 2nd, or 3rd tick proc)
All remaining segments have 1 second's worth within 4 procs, and 2 seconds within 3 procs. - 29.53%
The approaching limit should be 29.53% of 220, or
~65 spell damage on average

With talents, and 2 piece T6 set bonus, I get 9 ticks with this trinket.
Hmm so it Should proc once a swp about for me.
I agree this trinket seems really awesome.
On trash its godly at least. All the time on. Boss was a bit slower but without any numbers, I think its better than eye of maghteridon as example.
This+Darkmoon card makes awesome combo for bosses.
I think this is better than darkmoon card tho. Cause that has 10sec "use" time and then have to reapply. So loose alot dmg there often.
So this exalted trinket with the ZA one. Probably best combo there is atm?

The short duration of the healing buff makes this pretty useless.
not only does it "ony" have the same amount as the damage buff (normal for healing is ~2x that of damage) it has half the duration, making it kinda hard to get any use of the buff.
From the time it takes to realise u have it to the time u cast something with it it can very well fade...

used this trinket duration of full hyjal (including archimonde) and procrate was 5.67% on total...
i got it, not using...
edit: as shadowpriest.
edit2: and best combo for shadowpriest is illidan trinket + ZA trinket.

Why oh why are you guys arguing if its shadow or holy. It says clearly priests. and Second of all its not a drop instead a vendor item so pick it up if you want it. I say it can be worth it in the 25man raids where damage over time to groups are common in TBC. When you're not throwing renews around just swap it out for another trinket. Same goes for dps. Enjoy the trinket because it can become insane.

this is a pretty terrible trinket for shadowpriest overall, the only time u would use this hunk of junk is when ur bored and decided to dot up all that annoying hyjal trash.
Best Trinket Combo currently is Timbals+Hex.
Course Hex has been taken out of the game so the true killer combo would be Timbals+Skull but that seems to be taken out of the game as well.
Too bad.

If you have a single renew or SW:P up constantly, this item will give you an average of 35.2 damge or healing.
Math:
ticks every three seconds, but lasts five seconds. So at any time, if you're in the first two seconds after a tick, you have a chance to be procced from the last two ticks. If you're in the third second after a tick, you can only be procced from the last tick. So 2/3 of the time, you have a 19% chance to have it. 1/3 of the time, you have a 10% chance to have it. That averages out to 16% (weighted by duration). That times 220 gives 35.2 damage or healing.
That's assuming that you ignore the procs and just continue to cast regardless of whether you have the buff or not. If you cast more while you have the buff, the average goes up.

As a holy priest, I'm sticking with my
Blue Dragon card
and my
Earring
. I like Renew, but this trinket just doesn't hold up against some of the awesome alternatives that raiding Holy Priests have.

Actually its not that good.
Only proccs on renew -heals-, and since you normally have CH shamans, FoL pallys and maby even lifeblooming druids, you get very few ticks that heal.
Ofc theres exeptions tho, like on the RoS fight (P2) where you can renew half the raid and basically have a permanent +220 healing.
Another big disadvantage it has is that if it somehow proccs, and you want to refresh that +220 renew/SWP (maby before a boss casts silence or fear or whatever) you cannot (stronger version allready active etc etc).
This trinket should IMO be tuned to a on-use, or maby even some passive bonus.
Maby even have a chance to procc on all heals.
As it is now (with ilvl 141 and all) even a rejuv gem is far better overall (ilvl 75..)

Somehow I don't see those stats standing for very long.... we're supposed to get more + healing from gear than + dmg, and why does the +dmg last 10 seconds, and the healing only 5?
Overall tho, this is pretty rockin.... I'm a holy priest, and if all the renews i have out on various members of a raid have a 10% chance to proc for EACH TICK.... I mean, 5 seconds isn't much at all, but i'd take it.

Eh, why does the healing portion last half the time of the damage portion. If it procs every 30 seconds, and lasts 5 seconds. It's only going to be on a small portion of the time.

My guess would be because during a boss fight you might have multible renews ticking, but you're likely only to have 1 shadow word pain..
Not to mention holy priests could just slap a rank 1 sw:p on the boss and continue healing, but you can't cast renew while in shadowform

For shadow priests in groups this trinket beats any other trinket in game. Against single mobs it's on par with other epic trinkets. With the T6 2/5 bonus and fighting Hyjal trash the bonus is near permanent.

Nono, Hidden cooldown, as in not stated.
For instance, the Lightening capacitor has a 2.5 second hidden cooldown on its charging ability, which nerfed its use by scorch and arcane missile mages.
