Long story short, there is but
one goblin
, faithful to Booty Bay but residing in the Badlands, who gives Bloodsail rep through Revered. Each kill is worth one point. One point per kill, for one particular NPC.
A non-human would have to kill that same NPC 20,999 times to hit 20,999/21,000 Revered with the pirates, and needs the foresight to save their quests until they've hit that cap, so that the quest rep can push them into Exalted. In the comments to the page linked above, Wigwam (mentioned in the previous comment) states that he farmed this NPC for ten months to become Exalted with the pirates. If that's not a Feat of Strength, it should be.
Edit 28 Oct 2011:
The above post was written in response to somebody asking why "Insane" only requires Honored with Bloodsail, and whether it was possible to be Exalted among the pirates. Prior to the Shattering, Honored was about as high of a reputation as one could reasonably get, and it involved an incredibly challenging battle royale with the Booty Bay Bruisers. However, it was possible (if not rational) to become Exalted with the Bloodsail, as I detailed above.
As of Patch 4.0.3, Booty Bay Bruisers now give Bloodsail rep all the way to Exalted, making the full rep grind considerably easier. There's still no reason to go beyond Honored, but there's not much harm in it either, since the goblins already hate you.

you likely do not need them all at the same time. the other rep based achievements that require 20/25/30 etc work on a bar counting system (similar to rep itself really) however this achievement works on a milemarker type system. so when you get honored with bloodsail for example it should turn green and stay such. just like doing the oracle/wolvar or any other achievement with a defined list. if i hadn't already trashed my bloodsail rep, a whole week before this achievement came to fruition, i would test it.
another good example is that horrible holloween mask achievement. you don't need them all at once you just have to have owned them at some point.

Hello,
Yeah started doing this achievement, or rather prepare for it when it hits Live.
Buccaneers
rep grind is extremely easy and rather quick (<10hrs) even if you are hated like I was.
Ravenholdt
's first part is easy but for the 2nd part (lockboxes) you really need an alt rogue or a friend whose willing to grind those boxes for you. If you dont kill the target you have pick pocketed it will take around 10mins or so before you can do it again, so this kinda takes the low level alt rogue pick pocketing out of the question unless you want to change the place often where you are pick pocketing.
Few things that makes this a bit easier/cheaper (trying to avoid AH like a plague, unless the stuff needed is extremely cheap for some weird reason) -
Inscription -
Making the decks for the Darkmoon rep, scroll of recall for the libram, diamond, goblin rep grind in DM North. So everytime you've completed the quest and gotten at least 1 libram its a good idea to HS/scroll of recall back to Camp Mojache, sell the crap from your inventory and send your libram to your alt since as its unique you can only carry one at a time. And when you are turning in the librams unless you are engineer/got engineer friend with the portable mailbox
Enchanting -
So you can dis all the loot for the shards needed in the libram turn in quest.
Rogue -
For pick pocketing the lockboxes needed to raise up your Ravenholdt rep, DM ogres are a good target.
Skinning / Leather Working
- For the Darkmoon armor kits.

Confirmed on 3.1 PTR that you can be higher than honored with Bloodsail and it will still count towards achievement
Also it will append the title to the end so it will be "Lostshard the Insane"

and then do the boot bay grind again?
In all likelihood, yes. Aside from Mercenary of Sholazar, all of the multi-faction reputation Achievements so far require that you maintain the stated level of rep with all those factions at the same time. For example, if you became Exalted with the Aldor and three other factions, then, without doing anything else, worked your way to Exalted with the Scryers (thus trashing your Aldor rep), you wouldn't get the "5 Exalted Factions" Achievement because you're only exalted with four factions.
If this is anything like those, you'll need to destroy your Steamwheedle rep, then bring it back up without bringing your Bloodsail rep below Honored (which disqualifies the cloth turnins; you'll have to either zap giants or run Dire Maul). You seem to be forgetting about
Hero of Shattrath
, which is also a Feat of Strength, and tracks your rep status based on whether you are exalted, any time after the patch hit. (Unfortunately I hear it didn't track based on whether you learned a pattern, but that's irrelevant here, aside from indicating you MUST hit the rep level after the patch).
Therefore, it's likely you will have to do the Bloodsail rep grind, but not both, if you're currently sitting at exalted with all Steamwheedle.
However, Blizzard may decide to require you to be truly insane, and raise both to that level, as described previously in other comments. One must note the wording within
Hero of Shattrath
vs. this achievement:
Gained
exalted status with The Scryers and The Aldor.
(note the past tense)
Raise
your reputation with the areas listed below.
(note the present tense)
But on a counter-argument,
Mercenary of Sholazar
also uses present tense:
Earn
exalted status with the The Oracles and the Frenzyheart Tribe.
I guess at this point the best that can be said is:
Only time
(or PTR testing)
will tell.

you can repeatedly turn in cloth to bring up your reputation to neutral
You
can
turn in cloth to gain rep, but it probably wouldn't be a good idea for this achievement. As of 03/17 it's not known if you have to have all reputations simultaneously, if that's the case you loose 500 rep with the Bloodsail Bucaneers with every cloth turn in.

quoted from Phyloni
So someone who already put alot of time in it has to spend more time than someone who didnt? Doenst sound fair to me?
Things are not always fair
Timbermaw Reputation was nerfed 4 times since its release
1. Removed the only quest to get to exalted and added in beads/feathers instead
2. Increased the rep gain of beads and feathers.
3. Increased the rep gain of kills/
4. Increased the rep gain of beads and feathers.
So now you can get exalted at 80 in 4 hours now... and originally before 4 nerfs/buffs(depending how you look at it) took weeks. So is it fair for those who did it before nerfs? Not really but now timbermaw grind areas are not overcrowded with people and people can easily get to the diplomat achievement.
Insane in the Membrane is a Feat of Strength which means Blizzard does not expect you to grind it since its harder than normal achievements.
Edit: Xnub who provided the footage for the video did have to grind his rep twice
Edit 2: Maaven (a blizzard poster) said:
You'd be insane to try

OK, i answered my own question, or just found a solution, farm for more than 30 hours with goblins
The easiest way for a mage is to grab another mage, make a macro
/tar southsea
/cast ice lance
then go to the pirates south of ratchet and run around spamming that macro, with friend mage following you, when you go out of mana, switch places with the mage, run around for hours and youll get 6-7k rep an hour
happy hunting
EDIT: spelling
EDIT2: got rid of /tar baron in the macro, it messes up, when you kill him it will only target his dead body
EDIT3: better way than spamming, set it to scroll, and run around scrolling, lots easier imo

Do keep in mind that when grinding pirates for SteamWheedle Cartel, their respective cities (those you get 5 rep with per kill) will no longer grant ANY rep after achieving revered. They still will give 2.5 rep with all others though.
To get exalted with all the steamwheedle cartel, I grinded the pirates south of ratchet until Gadgetzan was exalted, and then switched to the pirates in the cove in Tanaris to finish off Everlook and Ratchet. Lots of grinding, but it goes fairly quick. I did this as a holy priest, wanding the Ratchet pirates and Holy fire/Smite'ing the Tanaris ones.
Putting stand up comedy on in the background or having a fun chat with friends on Vent makes the hours fly by.

Blizzard better put some insane mount as reward beside the title to reward something so dedicated players. The title for me wont be enough, I dont need only confirmation that I am insane for doing this achivement, I know allready that I am ;D Dont expect nothing fancy like Frost Wyrm that will get gladiators in this season but something crazy and unique like Anzu mount or something like that. When you consider how much gold we need to spend on darkmoon faire reputatation and how much time we waste for getting other reputations to exalted I think its something that people deserve for getting this achievement.

That's because scribes, for now, only make them to level. Once we figure out that they're for an incredibly desirable achievement? Through the roof, baby, through the roof.
On my server one guy is already spamming trade for level 60 cards and decks, stocking up for the patch. Watch the mats for all levels of cards go up too, like thorium widgets and the other stuff did back in the day.

Here's a small guide I wrote to help Rogues with some farming of lockboxes for the Ravenholdt rep grind. Credit to
panicka
and
herrojima
for educating me on LBRS vs BRD in terms of droprate, mob level and humanoid population.
Note that this is a speed & efficiency guide for rogues level 65+ and omits trash selling of looted greys (like books), food, Vanilla WoW gems and health potions etc. I destroy those items from my bag since the boxes are what I'm after and additional items like these tend to slow you down. I use level 65 as a reference, since at that level; you shouldn't be caught too often or at all regardless. If you're still having problems with being caught often,
some
points
in
Subtlety
and/or some +Stealth gear should help you out immensely:
Get 18 or 20 slot bags. You're gonna need all the space you can get. 20 is preferred. Spend the $$ and upgrade your bags if you can-the money will come back to you by the time you've done a sale of a few boxes.
Empty them all. You're not going to be fighting anything in here. Killing a humanoid will yield less loot. If you're caught/pickpocket resisted/detected: Vanish.
Keep your Hearthstone and a stack of Health Pots in your bags. You shouldn't need anything else. If caught - Vanish.
For maximum efficiency
- set (Horde) Hearthstone to Kargath. Sorry Alliance, I don't know your FP's. :(
Turn on
Auto-loot
in Interface options. You'll be one-button target/tapping all the way around.
Make a
macro
to Sap/Pickpocket mobs and bind it to a hotkey.
Suggestion -
I use
'Q'
since I move with WSAD.
Start from the front of LBRS and pickpocket your way around. Don't worry about getting into UBRS or killing the various mobs to open the doors. We're going for speed + time VS profit here. Clear the UBRS entrance room and keep going around.
PP your way through the instance up to Wyrmthalak at the end. If you still have bag space by this time - log out while in the instance and log back in. All pockets will have reset, you're standing next to the end boss so it's a matter of reversing your direction now - PP your way
out
now and you should have full bags by the time you've reached the entrance again.
If bags are full: hearth out to your nearest FP, head to a vendor and start opening those boxes. Remember to turn
OFF
Auto-loot before you begin or you'll lose everything from your boxes when they're opened. Remember to leave
at least one item
in them. Glyph of Pick Pocket (-100% cast time in Patch 3.1) practically shines here by reducing the time needed to open each box. Various vendor trash calculation mods are helpful here to see what's the highest seller of what items are in them. Congrats on any greens/blue/epics you may get in a box or two!
Using the mailbox at your nearest FP: mail all the boxes in bulk to your alt/bank character/customer, get back on the flight and head back to Thorium Point to begin a fresh run. Pockets will have respawned by the time you get back so you should be ready to collect again.
Happy pickpocketing everyone and good luck on your
achievement
!

I did this before it was considered an achievement, simply because I wanted to exalt every faction. it took me nearly a year to grind every exaltable faction to exalted. With that, here are a few tips I encountered.
THINGS TO NOTE BEFORE YOU START THE GRIND:
-If you plan on doing
Loremaster
at all, do this first before ANY GRIND, esp the bloodsail or steamwheedle grind. Quests for the steamwheedle faction give negative rep to bloodsail and took me from revered into mid honored when i did it. Had i not been revered, i may have fallen to friendly and had to go do extra work.
-Shendralar can be a rough grind if you dont have the pristines. I happened to have a few hundred from when I was 60 and bought them super cheap in hopes they would sell better one day. Now they sell at 50g+. One thing I would suggest however is that since you have 3 choices for librams, I would personally do Librams of Focus. The reason I would choose this (unfortunately in retrospect) is that any class can
solo farm Rivendares mount
. I got mine in 63 runs. During these runs, you have to kill a lot of abominations which drop a lot of frayed abomination stitchings. You can easily get a stack of 100+ abom stitchings and not have to worry about that part of the grind. in addition, enchanters can disenchant the items dropped in strat and have the required large brilliant shards as well.
-
ALWAYS do the bloodsail grind before steamwheedle
. I made this tragic mistake because I wanted to exalt any faction that could be exalted, and at the time, it was believed that bloodsail could only go to max honored. Plus I already had friendly/honored with those factions. Therefore, I full exalted all of steamwheedle, then did bloodsail which took all of those to full hated, and then had to grind them all back to exalted.
-
Troll the auction house
. This achievement is as much about cashflow as it is about wasting time. Neither of my professions helped for this grind, so I had to buy everything. When i did my grind, I had a list of items to look for on the AH and searched multiple times a day. In doing this, you can find cheap darkmoon cards for old world decks, librams, large stacks of shards, the occasional junkbox, rugged leather, etc. It took me about 9 months to buy all the cards to fully exalt darkmoon, and I never spent more than 300 for a completed deck, and usually I bought cards at less than 25g each. Elementals and Warlords are the decks you should focus. Old world decks are limited by the drop rate of the Ace, as it is the rarest card and always comes off a set mob/set of mobs. The warlords Ace drops from High King Maulgar, which is conveniently in DM North, which you should be farming for keys anyway. The elementals ace drops off many mobs, so it wouldnt be a problem to solo farm either.
-
Buy a Travelers Tundra Mount.
If you have the money, I would highly suggest buying it. It pays for itself in time. With the ability to zone out of an instance you are grinding and vendor everything you just shift clicked, you can easily make 20-50g per instance run. I made over 2k one night when grinding stratholme for the Rivendare mount. It is also much faster than vendoring trash in the library of DM north. It can easily make your DM runs go from 4 to 5 per hour. Also, the mount sells the thread needed for Gordok Ogre suits. Otherwise you will find yourself making a long run back to a vendor in Feralas.
-
Go into DM north prepared
. The keys in DM unleash a cache of leatherworking and tailoring supplies, which happen to give most of the materials for Gordok Ogre Suits. Keep in mind though that the materials in the cache are random, and although they can give Rugged Leather, you will tend not to get 8 rugged leather per cache. It helps to have 8 leather per run however to make 1 suit per run. Therefore you may want to buy a few extra stacks before entering DM. Also, make sure to buy your thread and get all Runecloth that you pick up along the way converted into bolts of runecloth.
For the Darkmoon Grind, you will need a &*!@ ton of money.
For the Steamwheedle grind, you will need a *!@# ton of time.
For the Ravenholdt grind, you will need a $%^& ton of Red Bull, stamina, and the ability to sacrifice personal relationships.

Many Steamwheedle quests don't decrease Bloodsail rep, right?
Are these
Booty Bay quests
, some of which also appear on the other Steamwheedle cities' pages, the only ones that decrease Bloodsail reputation? If I want Loremaster, should I just do these and
then
grind Bloodsail, minimizing the amount of times I have to do the Ogre suit quests, or should I do Loremaster first?

Well, the thing is, while working on Bloodsail, you will lose reputation with all Goblin cities. So it's better to work on your Bloodsail reputation first, then Booty Bay and the other three.

Ok, so this is an alternative way I used for the Steamwheedle reputation, bear in mind that I tested this and it works, it also seems to be faster than farming DM North from Revered -> Exalted
So first of all the theory for this; I figured that (assuming you're at 0/36000 hated) you could start off farming Ratchet up to revered on the pirates, its pretty common knowledge by now that you'll get it at twice the speed of the spillover rep. When Ratchet hits revered you should be at 31500/63000 rep with the rest (63000 is the amount from hated 0/36k -> revered)
Now, what I do at this point is I move down to Gadgetzan and start farming the pirates there instead, once again common knowledge, Ratchet will now continue to go over revered at 2.5 rep per mob. So obviously, you now farm Gadgetzan untill it reaches revered. When Gadgetzan hits revered you should be 15750/21000 with Ratchet which means you're only missing 5250 rep untill it hits exalted, then just go back to Ratchet and farm the rest untill they catch up with it (Ratchet mobs are capped, you won't get any more rep with it up there you're just evening it out now).
Now for the last 5250 reps im gonna split it up into 2 ways since I realise not everyone is in the same situation as me, being a person who haven't done ANY quests with Ratchet, Everlook or Gadgetzan on the character earlier.
Way 1: If you have done the quests in Ratchet Everlook and Gadgetzan just skip down to the next paragraph, since this won't be possible for you. This method is simple, you just go to the towns and do quests there, this gives approx. 5000 rep. This means you'll need a total of ONE key run in DM north for exalted. Only condition is
be 100% sure that you are NOT doing a quest where Booty Bay is the main faction, if its tagged Gadgetzan Everlook or Ratchet, its ok, if its tagged Booty Bay, DO NOT DO IT, if its from the area but he's not tagged as any goblin faction, check this site or a similar one on the quest and see if you get most rep with Booty Bay, if you do, don't do the quest
Way 2: Its the basic method, just go to DM north and farm keys, 15 runs without ogre suits, 12 with.
Now the rest is just napkin math I did to check it, I've used this myself and can confirm that its worth it.
Revered -> Exalted = 63000 Reputation
63000/2=31500/2=15750
21000 - 15750 = 5250
5175:425 = 11.94 (Note that its less since I count in the extra rep from first time you turn in the suit)
5250:350 = 15.00
All I had to say, cheers

There is a great section on how to raise rep with the Cartel here on Wowhead and yes, this tactic works. Basically; the goblin town you are closest to while killing pirates gives more rep per kill than the other towns.
So, when you reach the limit on one town, relocate to another area near to a different goblin controlled city and get the most bang for your kills. Read more on this on the in the
Booty Bay
section of Wowhead
Also, here's the exact play-by-play on how to build rep with the Cartel without losing
Bloodsail
rep, by
Marivel
Southsea Freebooters
: Give 5 Ratchet Rep until 11999 honored. 0 Ratchet Rep after. 2.5 spill over rep to all others through Exalted. Named mobs work the same except 25 rep and 12.5 spill over. (These pirates can be found along the coast south of Ratchet and north of NorthWatch Hold)
Venture Co.
: Give 5 Booty Bay until 11999 honored. 0 BB Rep after. 2.5 spill over rep to all others through Exalted. Named mobs supposedly work give 25 rep and 12.5 spill over, but I never found any. (These mobs can be found throughout STV, mostly west of Zul'gurub as well as East and slightly north of the Gurubashi arena, near a cave)
Wastewander
: Unconfimed rep at friendly. Do not give Gadgetzan Rep past Friendly. 2.5 spill over rep to all others through Exalted. (These pirates can be found in the desert east of Gadget)
Southsea Pirates
: Give 5 Gadgetzan rep until 11999 honored. 0 Gadget Rep after. 2.5 spill over rep to all others through Exalted. Named mobs give 25 rep and 12.5 spill over. (These pirates can be found in Lost Rigger Cove in Tanaris)
Venture Co. Evacuee
of Venture Bay in Grizzly Hills: As far as I can tell, these work the same way as the Venture Co. in STV. Take longer to kill, same rep, but they do drop loot from Northrend, such as Frostweave Cloth and various grays and greens between levels 70-73.
Again with the Zapped Giants Repeatable gives 25 rep for all Goblin Factions no matter what your rep is at.
Refuel for the Zapping Repeatable gives 25 rep for all Goblin Factions no matter your rep.
I have not found any mobs to give 5 everlook rep and spill over to the rest of the goblin factions.
This information is from a level 80 (Holy Priest) standpoint. None of the rep gained from these mobs will disrupt your BloodSail Buccaneer rep.

ofc if you arent a rogue you need to level one, and this will add another 160hours (leveling rate taken from 2 sites selling leveling service)
160h lol... you dont need 160 hours to lvl to 50-60
its easy to get to lvl50 in 2 days or 2 days and a half @played
if we talk about leveling to 60, it should take you one more day. that means 3 days @played aprox.
160h means 6.6666 days... if you are not quite used to leveling and you are not using any help (see brian kopp / johana mancow guides), it could take you even longer... but omg, i leveled my warrior as prot@pre3.0 (without sword and board or shield slam) and i spent 7 days... been an Absolut noob and spending almost all the time thinking about what to do next without any real idea of how the game works.
lets imagine this rogue is your second character and you leveled your first char years ago so you dont remember how to power level your character quite fast. in this situation im sure you'll spend 4-5 days leveling to 50-60 (5 days to 50 is Really easy). rogues are the fastest class to level if you know how to level them. if you dont know, they do nice dps and they are not hard to lvl at all.
that means you could spend 100h as an average if you are not used to leveling, or using a leveling guide plus questhelper/carbonite addon, 70-80h.
if you think any of this is wrong, just reply and tell what you think. all of this is based on my personal experience, so it could be absolutely wrong... or just a little bit mistaken. who knows! :P

For those who are beginning to think about this grind, be warned:
It will take a very long time
. I have been tracking my grind for it on my blog site,
http://www.insanewillpower.com/
and have reached maybe
1/10th
of the total grind, if that. I've done all the easy ones, such as grinding Ravenholdt to revered, Bloodsail to 11999/12000, and Darkmoon Faire to 2000/3000 Friendly and I'm already looking at
25 hours 35 minutes
of farming (yes, I'm actually timing it). Feel free to check out my blog though, as I leave various tips and ideas there, including examples of farming paths and statistics from actual experience.
Also, if you have the gold, buy yourself a
Traveler's Tundra Mammoth
. Trust me, it makes THE WORLD of difference sometimes.

I would have had this Feat finished long ago, but I decided to hit exalted w/ Bloodsail instead of stopping at honored. There should be something for that. I mean, the exalted Bloodsail grind is a difficult task in its own.
http://www.wowarmory.com/character-reputation.xml?r=Blackwing+Lair&cn=Alterraia
Oh, and I started my Bloodsail grind about March 6th. I finished and hit exalted on April 16th, and finished this particular feat on May 10th. So, take how many days it is between March 6th and May 10th and subtract about 45 days (40 for the grind from 1 point under revered to exalted with Bloodsail and 5 days bc I took a break), and you'll figure out how long it took.

Just a thought, if you have
NOT
done Loremaster or have not done many quests that yield Steamwheedle Cartel rep. you should probably hold off on them till after you are done farming pirates to gain back your Booty bay rep (seeing as the majority of quests give a rep of some sort).
I'm slowly working on Loremaster and have been thinking about trying for this feat of strength and one day while working on Loremaster of Kalimdor I did a quest in Desolace giving me rep with all of Steamwheedle Cartel. I didn't think about it before hand till I had turned it in and got all the goblin rep >,< now, I'm not sure if you lose Bloodsail Buccaneer rep for doing quests yielding Steamwheedle Cartel rep since i'm 0/36000 Hated with those pirates. One would think you wouldn't since from what I've read so far, is that you lose rep from killing Bloodsail Buccaneers. But this is just a thought, hopefully its of some help for those trying to get their goblin rep back up.

I just completed this today, took about 10 weeks and 30k, totally worth it. Found that Shendralar was the most difficult of them due to the short supply of librams on the AH.
I levelled a rogue to 53 and had no problem with pickpocketing in Tyrs hand as Sub spec, you can clear out the lower lvl of the place in the time it takes for the pockets to reset - the cathedral building at the higher lvl has mobs 57+ so I avoided this area. Be careful not to die in one of the chapels tho, as theres a current bug which leaves you stuck in DK area til a GM can get round to you. There is also the odd rare 59elite spawning, just log your main and kill em.

Something to add before various people go mental on the Ravenholdt grind:
I tried Alterac Mountains
Arathi Highlands
and Durnholde Keep(with a ret paladin doing the same)
for the kill grind.
AM was around ~2.5k, AH 2-2.5k rep per hour.
DK we duoed 12k rep in exactly 1.5 hours - thats
8k/hour
yes.
Paladin took the lower area with the houses, myself as warlock circled around on the upper area.
I strongly suggest for anyone that needs to do the rep grind, either go there solo and do the lower area of DHK or take yourself a 2nd guy that does the grind and have even faster clearing.
Also, in the case of being enchanter:
/cast Disenchant
/use Dusksteel Throwing Knife
/script AcceptQuest()
/script SelectGossipAvailableQuest(1)
/script CompleteQuest(1)
/script GetQuestReward()
This will ease your life greatly.

Highly suggest farming the pirates in Venture Bay in Grizzly Hills, fastest respawn of all the places and the gold you gain is quite a lot (Avg 52g a hour atm). I'd say start here until exalted with all but Booty Bay then go south of Ratchet to farm exalted with Booty Bay. Get the most gold, and probably the best rep per hour. (Especially if you're in a group of 2+). Also great if you have a traveler's mammoth or have access to one, you'll make a killing off the greys/greens.

its a good advantage to be a human for this achievement as you get Diplomacy (10% extra rep) also a easy way to avoid losing rep with blood sail buccaneer's is to go to dust wallow marsh if you run out of places to quest... i did that from 35-42 on my human warrior and then ofc screwed up and lost rep on the escort chicken quest in Tanaris...

Well folks. I am almost done.
I am getting out of hated with Steam Cartel today. I would assume another 2-3 days of DM runs and I will have this stupid title! Insofar as gaining rep with Steam Cartel -- while killing the individual mobs may be easier out in ratchet and tanaris, DM is much more beneficial if you are an enchanter (which of course I am). I have made close to 5k-6k with the mats that I have gathered from 0/36k hated to Hostile.
I have to say that without a Rogue alt or a friend that ABSOLUTELY loves you, you will not be able to get through the Ravenholdt rep, therefore making the title impossible to get.
I will update when I have completed it.

I forgot if I posted this method, but I'll do it again just in case.
My way of farming Darkmoon Faire rep was simple. When the Faire was live, I stayed in Shattrath City (it was outside of there in May), and I had a macro that said something like "Paying 50g to turn in your Darkmoon Faire deck for you. The trinkets are BoE so you can get them right back. I'm only after the rep." Towards the end of the week, I upped it to 100g per turn in and I got MANY decks from this.
It sounds fishy but if you were scamming them and they weren't stupid, they could report you. A good way to convince them is that you mention that they can continue to sell the trinket when the Faire is gone, giving them an advantage over others who are stuck with the decks. Most players don't know the trinkets are BoE and many dont' know the difference between the trinket and the deck.
I believe this coming Faire in June is outside of SW, so if you're Horde, I suggest hiding in the water in SW so you can have access to trade chat.
This method saves you a lot of farming for low level decks and players will more than likely be after that free gold they get for letting you turn in their deck.
Some players will catch on and offer to sell the decks for a lot more. I was the only one doing it on my realm so I had the advantage, but unfortunately, I had random players saying I was scamming them, and I had no clue who they were, but thankfully the players who I did pay to turn a deck in for, they stuck up for me.
Oh, and one last thing. Be VERY careful with a Nobles Deck. I tried to avoid those honestly. If someone allows you to turn that in, make sure you double or even tripple check which of the four trinkets they want because more than likely, they will be stuck with whatever one you pick, and a GM might not be able to assist them since they were not the one who chose the reward. But then again, considering the situation, they might be of some assistance.
I hope this helps. : )

After a long & painful slog, I finally achieved this 20 minutes after the fair spawned in Goldshire. It was a long running race to be realm first with the guild leader of the top alliance guild hot on my heels. At the end of the day my OCD paid off & I got there first. I won't go into the details of the grind as they've all been well described in detail. All I can say is that if you are planning to do this. Be veerrrrry patient & hope that your guild & more importantly family & friends will be patient for you.
If you do get this achievement, I honestly believe you will have achievent by far the most coveted title in the game. All these heroic title just prove you have powerful friends. This title shows you are about as dedicated as you can get.
I kept a full log of the times I took, best places to grind. If anyone wants advice on doing this I'm more than happy to help.

As someone started to point out Dire Maul runs give the chance to drop librams. Also then consider the fact, that at the least, if you were able to buy all 3 librams, you would still need to make close to 30 dire maul trips anyway, just to turn them in.
Some math: To get reverred with Ratchet/Tanaris pirates you need to kill 42000 pirates.
(36000+3000+3000+3000+6000+12000+21000)/2.5 + (21000/2.5)=42000
Also, not even mentioned is the spawn rate. Especially in ratchet, when I attempted it, I was beating the spawn timer frequently.
In the end though, my suggestion, is to gather up all the mats needed for turn ins for librams (I started by farming 20 of each libram worth of mats) and just carry them with you in DM. As they drop you turn in them. And for me personally, getting 84000/350=240 keys to drop isn't as bad as killing 42000 pirates. You can also lessen the number of keys by carrying the runecloth bolts, leather and thread with you. There is an ogre tanin in every dire maul, and each key box can drop them. So you can surpass the previously said 2175 rep an hour.

I'm working on this now (lock) and am doing the DM runs for exalted goblin rep. I can d/e as well, so I can sell the mats on the AH. But the easiest way for me, I dragged my bf (warrior) along. We'd kill everything until 1-2 keys drop. I take the 1st key drop, free the goblin, then go back and get the 2nd key. Reset, repeat...
I tried the ratchet pirates, but running around in circles make my head hurt, so I stop. Plus, all the lowbies who are there that actually need the mobs... Tried Tanaris and same thing w/ running around in circles. =/
I guess it's just preference...

As being on a Oceanic Realm - for the past week I've been working on getting the Darkmoon Faire reputation up to Exalted. I failed to done so but - I was only four decks away til Exalted.
Seeing another method of getting this reputation up; I offered people every day on trade literally spamming my macro every 3mins to offer 100g per deck turn in. Now in the first days, it dragged much attention and spam that I'm a scammer etc but luckly for me some people don't understand the term defamation - which brought a easy GM ticket.
Whilst doing the Darkmoon Faire reputation, the NPC which offers
https://www.wowhead.com/wotlk/item=19422
can give you 75 Reputation (4/29 chance) which can boost your reputation slowly if need be.
So in the first days I offered 75g, then 100g and I had many people craft decks and help me. All I done for the macro was:
"xxxx is going for "Insane in the Membrane"! Assist me by letting me trade in your Darkmoon Faire Decks! I get my rep, you get your trinket plus 100g per deck/s! PST for info :)"
I had to explain the achievement many, many - MANY times and cop with spam which was fine for some nice people who know that this is. This achievement is very worth it and I ended up on 19617/21000 Revered and most likley just use
https://www.wowhead.com/wotlk/item=37163
to hit Exalted.

Finally finished the goblins a few days ago. I personally have to go with Dire Maul if you're working on Steemwheedle. I've gotten about 75/84 of the librams I need along the way and I'm confident I can find the rest on the AH.
Pirates north of Ratches definately gives faster rep but it gets insanely (lol) boring fast. The respawn time is practically instant in this area and you can kill Baron Longshore for 25 rep 3 times within a minute if you catch him. I dug myself out of Hated for Ratchet here before I went to Dire Maul.

The following link is an extensive guide on how to obtain the "Insane in the Membrane" Feat of Strength which rewards "the Insane" title.
The guide is approximately 7700 words and encompasses every aspect of obtaining the title.
FoS Guide - By Callahan of Ysera-US
The guide is as accurate as I was able to make it. Any mistakes should be pointed out here, or by whispering me in game.
Please rate up if this was of any use to you!
I found a few errors in this guide in section 2.2.2 – Running Dire Maul as follows;
Don't bother killing King Gordok, hes useless. He is not a guard so he doesn’t drop the key.
He does drop Librams, and items which can be disenchanted for the quests of said Librams.
You can't sneak between the wall and the ogres by the Ogre Tannin Basket. It is not possible without stealth.
This information is inaccurate if said player character is level 80. The player simply should hug the left wall from the ramp. From the left wall the tannin basket would be on the player's right, which is easily lootable without aggroing the ogres.

I can confirm that currently (June 17, 2009) you DO NOT need all factions simutaniously.
http://www.wowarmory.com/character-reputation.xml?r=Hyjal&cn=Ny&gn=Advent+Fury
As can be seen on my profile my Bloodsail is a bit into honored (want to fill at least the Honored bar up anyways, then ponder the meaning of my life and the meaning of the life of the goblin in Badlands/that hidden harbor place) and my goblins are completely bottomed out, but I just got my Insane title yesterday.
I was exalted with the goblins when I started on Bloodsail Buccaneers a couple days. You can see that I have the Exalted title on Ny and my paladin, Nya, but they are at 39 and 36 exalted reps after thrashing Booty Bay. Last night I hit honored with Bloodsail and was unexpectedly hit with the achievement announcement. Actually kinda made it really anti-climatic. No build up of excitement, no chance to start singing Cypress Hill in guild right before my last kill. Granted I wasn't exactly looking forward to doing the goblin grind from 0 -- doing all of revered was plenty enough fun... though we all know what rep-nuts end up doing anyways...
Well I'll have another chance to sing with my paladin!
(Nya's rep)
Didn't get a chance to be first to insanity on the server, think am about fifth or so (grats Branical, Sereke, Aislenn, and I thought I remember seeing at least one other running about), but maybe I can be the first sicko with two insane characters! Still quite a few librams and junkboxes to go.
Misc low/highlights of this insane adventure so far:
I see an apparently afk 80 NE hunter standing around Booty Bay, flagged (blue server). I suck at pvp so I hesitate, even with two characters, but not too much to lose when all the guards are *already* attacking me! So my priest melts his face. A nearby human sees this and decides this is his chance. He proceeds to teabag the night elf for a while.
Got one of my accounts suspended from the forums for joining in on someone's thread who was buying junkboxes for 1g. I was about 1000 junkboxes through revered Ravenholdt at this point, all farmed with my own rogue (who was leveled specifically for that purpose), so still needed about 500 needed for her and another 1500 for my paladin. Frankly, having read about and seen for myself the success rate in Tyr's Hand, BRD, BRS, etc I find 1g to be almost offensive. I post a reply that I'll pay 5g each, buying 2000. Woot high bid I think to myself. Granted 10k gold is an absurd amount of money to spend on such things, as I knew and was of course soon informed of by my fellow concerned community members, but hey, money = k * time. Your k may vary. I was also informed that I was a gold buyer and that "back in the day" you had to make a rogue yourself. Also, that buying your achievement didn't make it much of an achievement. I understand the idea, and it certainly applies I think to things like Gladiator and rewards that take skill. Insane doesn't take skill - just time, patience, willpower, and perhaps social issues and phobia to sunlight. I don't think I'd feel too bad about buying some junkboxes when I think back on and look forward to more riding around plaguelands for hours looking for blood, flipping strat/scholo/dm for hours, leveling a rogue, hitting ambush instead of pickpocket again, leveling inscription, mindlessly slaughtering thousands of pirates, etc. If anything "cheapens" my title it was not having to redo goblins from scratch and lucking out by only disenchanting 3 berserker cards before noticing they weren't bop anymore and cheerfully realizing I could get back most if not more than the 30000-40000g I'd spent on Darkmoon Faire exalted x2. I really do feel a little bad for the people who finished that one before the change. The truly insane are those (or that one guy) who did it pre-tbc or pre-inscription. But anyways, my "buying 2000 for 5g each" post got me suspended from the forums for a day for trolling. My response to convey my confusion and disbelief using my second account got that account banned from the forums for circumventing a suspension. Stubborn and not particularly bright, and still in disbelief at what had taken place, I bumped the thread the next day with my working-again first account. This got me a week-suspension and final warning against that account. Pretty funny overall, and no one even sold me any junkboxes!
Speaking of banned. My priest account was actually banned for "use of third party programs" last year, well before this achievement came out, but when I was working on Timbermaw and goblin rep in DM. I'm not sure which of these places triggered it, but apparently doing the same thing over and over for hours flagged me as a bot. Yet my paladin account, there with me and active the whole time, on the same computer, wasn't banned. Took a couple weeks and a lot of emails, but got my priest back, thank goodness. Of course as soon as I had both up and running I was right back in Felwood and DM again. "Didn't learn your lesson, did you?" my friend tells me.
While doing argent tourney dailies an hour or so after getting the title I get my first random tell:
Random: "Where did you get that?"
Ny: "My title?"
Random: "Huh?"
Ny: "Get what?"
Random: "Where do you get a lance at?"
=(
Guess better to sit on my troll's white raptor if I'm feeling attention deprived! =p

This Macro Will Save You HOURS!
/console set targetnearestdistance 10

/cast pick pocket
Set the first line to '5' if you dont have Glyph of Pick Pocket
Basically, have auto loot on and walk up to any mob and it will pickpocket it, if you spam this button while moving around a group of mobs it will pickpocket them all, very fast.
You might notice your chat log saying you've looted the same mob more then once, you really havent, it is just confused because you are changing targets so fast.
Just keep spamming the button, and it will pickpocket whichever mob youre closest to.
Have fun farming your heavy junkboxes!

It doesn't cost very much to get inscription to the level that you can make
Rogues Deck
so if you have an extra alt just make them an inscriber. Or if you're like me and never got around to making a death knight, make one of those and have them be an inscriber. It's worth the modest amount of money to reach skill 110 just to save having to bother your inscriber friends to make these for you.

Ok, this is what ive got for the darkmoon faire:
Rogues decks are approximately three mysterious tarots, and to make the 1440 decks youll need 4320 light parchment, and 2880 bruiseweed/stranglekelp. Why the herbs? It is because you need them to make the hunter's ink and midnight ink for the tarots. Also, for the daily darkmoon faire quests, the thorium widget quest needs 6 thorium widgets, which means for each widget quest turn in youll need 18 bars of thorium and 6 runecloth (easily farmable from stratholme), and an engineer friend or alt. For the quest armor kits where you turn in eight rugged armor kits you'll need to find a skinner friend, or if you do it yourself you can kill gordok mastiffs from DM which drop 1-2 rugged leather when skinned. 5 rugged leathers makes 1 rugged armor kit, so you'll need forty rugged leathers per quest turn in. You can also kill frenzied plaguehounds from the northern part of the Eastern Plaguelands which usually drop rugged leather when skinned (about 80% chance). i hope this helped.

Quick summary of my work so far, and what I plan to do.
Bloodsail
- Pretty straight forward. You have to select "At War" with Booty Bay and a couple hours of grinding later you'll be at honored. With the feedback from Blizzard, if you have decent Goblin reputations, I would get them to exalted first, then do Bloodsail. The Bloodsail grind goes very fast and should be one of the first things you do for this achievement. Would also recommend grinding your Goblin rep back to at least neutral as the 2nd thing you do, as it makes traveling a lot easier if you use any of the goblin towns for any reason. More on that below.
Darkmoon Faire
- From neutral to friendly you can do their repeatable quests, there are a LOT of them to choose from. Easiest for me was Rugged Armor Kits. After about 1300/6000 to Honored you will need to turn in decks of cards for rep. For this I had my DK alt pick up inscription, you need to be at 110 I believe to start making the Rogue cards. It's very quick to get inscription up, and much better than having to have someone else do it, because as mentioned you will need 1440 decks, which is 4320 cards.... You will also need a toon that can farm Stranglekelp/Bruiseweed, as they are the easiest and most abundant to find. I personally use Stranglekelp, and farm in the Wetlands off the west coast. Stranglethorn Vale and Westfall coasts are also great farming places. Also, Rogue decks/cards are rarely ever on the AH, so don't bother looking. Another option for those with a bit of money to invest, is to buy the lv. 60-80 decks of cards or make them. As each turn in is 350 rep. These decks range anywhere from 300 to 5000g, and the trinkets associated with them are now Bind on Equip, meaning you can buy the deck, turn it in, then put the trinket back on the Auction House in hopes of getting most of your investment back. You will need roughly 100 or so of these decks from friendly to exalted if you choose this route. Obviously most people will use a combination of the two, whichever are readily available. You can also camp nearby the Darkmoon Faire areas when it's in town and offer to pay people gold for them to let you turn in their deck for rep then give them the trinket back. If the Faire is at the opposing faction's territory, Mulgore/Elwynn Forest, you can still interact with the NPC's and turn in items like normal, just be wary of the opposing faction present. You can also easily sneak into Stormwind/Thunderbluff without much trouble to have access to Trade Chat to advertise offers for turning in decks for people, yet still be very close to the Faire.
Shen'dralar
- Really straight forward here. They reside inside Dire Maul in Feralas. To get rep you will need to turn in Librams that are dropped off mobs in Dire Maul (North, East and West) . These are Libram of Protection, Focus, and Rapidity. Each Libram requires certain materials to be turned in. Each will require 1 Pristine Black Diamond, a few Large Brilliant Shards, as well as one of the following depending on which Libram you have. Abomination Stitching, Blood of Heros or Skin of Shadow. The Librams are Unique items so you can only carry 1 of each Libram in your bag or your bank at any given time. Because of this, you will probably want to have a 2nd or 3rd toon on follow when you are farming Dire Maul for these, as you may see 3 or 4 of the same Libram drop in one run, and you would be forced to leave them on the corpses...
These Librams can be found on the Auction House, although rarely. When I have gathered a bunch on the 2 toons I use to farm, I mail them to an alt toon, then mail them back to myself and leave them in the mail for later. The Librams are turned in to Lorekeeper Lydros in Dire Maul. You will need about 86 Librams to get to exalted. I farmed the Auction House for the diamonds, and came across plenty of the shards while farming Stratholme for the Baron's Deathcharger.
Ravenholdt
- A long grind to start this faction. I recommend going to Durnholde Keep in Hillsbrad to farm the Syndicate there. You can kill them up to 11999/12000 just shy of Revered. You can farm about 4k rep an hour largely dependant on class. From there to Exalted you will need to turn in Heavy Junkboxes at the Ravenholdt camp. These boxes only come from pick-pocketing mobs around Lv 50-60. You must obviously use a Rogue for this. If you have a friend that will let you do this you are lucky, I leveled a rogue via "Refer a Friend" to accomplish this. I recommend doing farming circles in Tyr's hand in SE Eastern Plaguelands. Careful if you die over here, it can put you in the DK starting zone and you will need a GM to get you out. You will need 1405 junkboxes for 281 quest turn ins to gain exalted from revered...
Steamwheedle Cartel-
This is comprised of the 4 Goblin cities, Everlook, Gadgetzan, Booty Bay and Ratchet. If you are honored/revered to start with, I would recommend getting them to exalted first, then go do Bloodsail. I did bloodsail first, so my Goblin city rep's were all hated. On the flip side, you will be hated once you finish Bloodsail, meaning catching a FP to or from these Cities will be near impossible, so most will be grinding back to neutral after they get done with this achievemnt anyways. Two ways of doing this:
A) Farm Pirates close to the various Goblin cities (except for bloodsail for obvious reasons). They give 5 rep per kill to the closest Goblin town, and 2 or 3 to the rest. This method can take days upon weeks to grind, as most will have to grind 36k rep to get back to neutral with the goblin cities, if you had already done the Cartel quests, then you are in for a whole lot more grinding of these guys... It's the most boring, agonizing part of this achievement.
B) There is a goblin in Dire Maul North, he gives a repeatable quest everytime you do Dire Maul called "Free Knot!" . To do this you will need to find the key to his shackles. Practically any Ogre in DM North can have it on them. I read and saw that people say these keys are pretty rare and hard to find, and this is very time consuming. I disagree. Every run I have done so far I have found the key within 5 min, and often times find a 2nd key on my way out to reset the instance. This quest gives 350 rep to all Goblin cities, and he leaves a small crate on the ground with all sorts of goodies in it. he has a 2nd quest to make an Ogre suit, It gives 75 rep to all goblin cities, and is repeatable as many times as you have mats for. Obviously you will have to do this one before you release him. The "Ogre Tannin" is found in a small basket up the stairs from where you find Knot. You get 1 per run, so collect a bunch then bring in a ton of mats to do the quest a lot at once. The other big upside of doing your Goblin rep this way is the chance of getting Librams off mobs that you are killing for the key. So killing 2 birds with 1 stone, also all the greens can be DE'd later on for a nice profit on the mats. "Free Knot" plus "Ogre Suit" quests net me 460 rep (human), so all total I will have done this about 80 times to get back to neutral with the goblin cities, largely depending on how far hated you are after bloodsail.
Will continue to add more as I progress through this achievement and respond to feedback/questions. Thanks for reading :D

I have decided to run DM North in order to raise the Cartel reps. It's a very long process. The shackle key dropped about 80% of the time but expect some frustrating series of unlucky runs as well.
I am doing it this way for the sole reason that librams for the Shen'dralar rep are almost unavailable on the AH on my server. On top of that, they sell between 80g and 175g per at the moment. I don't have time to camp the AH for this and 80 librams times an average of 100g is 8000g (which I don't want to spend on this).
If you plan on doing this, here are a few tips:
a) Get a Kirin Tor ring: I ran back and forth from Dalaran for approximately the first 40 runs. I ended up wasting more time travelling to DM than running it... believe me, this is a good investment.
b) Get speed increase enchant/talent: As a pally, i took Pursuit of Justice as an example. You know it's going to be long but you don't have to make it longer on purpose...
This is for pallys doing it (how I'm doing it): I'm prot specced, naxx25 geared.
Enter the instance, down the ramp, kill Mol'gar.
Avoid patrols, go loot Fengus's chest.
Kill Fengus
Move on west and open the courtyard door
Avoid all patrols
Turn the gordok suit quest
Free Knot (don't forget to loot knot's cache)
Move on to the first floor and loot the tannin
Continue til you reach Captain Kromcrush
Kill Kromcrush but keep the 2 guards he spawns alive.
Run all the way back to the entrance, avoiding all patrols and groups of mobs
The spawns will bring around 30 mobs with them right to you.
Kill them all, loot, rinse, repeat.
Doing this, I often get one of the 3 Librams, a Pristine Black Diamond (rarely) and the Gordok Shackle key (often). Takes 12 to 15 minutes per run. I am also looting the Tannin every single run. You can get 1 or 2 more in Knot's cache for a potential of 225 rep per run just for the Ogre suit quest. It's also less frustrating when you don't get the shackle key. At least, you are still progressing.

This is totally wrong , i achieved this feat of strength while being hated by all goblin factions. I also achieved The Exalted title shortly after WOTLK arrived in stores. Apparently by doing this , it flagged them as being already achieved. Doubters , look me up on Exodar server , although i will still grind out both bloodsail and regrind all the Cartel factions back to exalted. I have proven it can be done otherwise than having them all done simultaneously.

Ah yes, 281 turn ins for Ravenholdt, 5 boxes a piece, I knew I wrote that wrong lol. I knew my original post needed some adjustments.
And as far as goblin rep and bloodsail and if you need them at the same time. I know there is some discrepancy being discussed but I think it comes down to "when" he was exalted with the goblin cities, as blizzard has not been tracking such things since release. If you are exalted now, it would be safe to say that you could get the achievement w/ goblins as hated.

you are all missing out one important thing considering the goblin cities and the bloodsails rep.
in front of all 4 goblin cities there is a traitor that gives you an unlimited repetitive quest wanting linen cloth, rune cloth and so on. heres the link to the traitor in gadgetzan:
https://www.wowhead.com/wotlk/npc=16417
if you do that quest you get 500 rep with that city and 250 rep for the other 3 cities BUT you also lose 500 rep with bloodsails. now why is that quest still good for the insane in the membrane achievement? its simple, dont farm bloodsail rep up to 1/12000, go for 11999/12000 instead. by the time you hit 1/12000 you are probably already fully on hated with all the 4 goblin cities so if you kill more of those you will just get reputation with bloodsails but no more bad rep with the goblins as you are their nightmare already.
now you can kill that goblin that gives 5 rep with bloodsails once and then you have 12000 rep from bloodsail to turn in for rep with the goblins because with 1/12000 you are just as honored as you are with 11999. lets put this in numbers:
12000 overrep / 500 reploss =24quests --> you can turn in those items 24 times and still be honored with bloodsails.
turning it in once gives you 1250 rep overall so thats 312.5 rep if you split it up equally and do all 4 traitor quests 6 times.
1250*24=30000 rep which is 7500 rep for each city or if you prefer gadgetzan in order to turn in that Wastewander Water Pouch, its 12000 rep for gadgetzan and 6000 for all the others.
its a lot easier to kill BB-bruisers for 12k bloodsail rep than killing a zillion lvl 40 pirates for 30k rep, but of course you can only do this once.

Currently, you do NOT need to be exalted with the cartel when it dings, however i would say you needed to be exalted w/ cartel at some point.

I probably won't worry about getting exalted again, although it's just one key turn in.

I just finished this last night and would like to contribute my two cents on a couple of things. This is not broken up by faction (as somebody already has a nice guide for that above), but rather sort of a non-linear way to think about what you need to do.
You have to be ready to spend some money to do this (or it will take forever), so the real key is to make some money as you go.
For the Bloodsail just group with other people doing this. Not only is it easier to kill mobs in groups, but more spawn when more of you are attacking.
For the Goblins, I spent most of my time outside of Ratchet until revered. Not only did this go fast, but I also helped power level a couple of friends. One note: even though we want to think about making money, it's not really worth looting here. Tell a friend or even a random stranger to just follow you, put the loot on FFA, and let them collect the exp and the loot. My toon is a Mage, and I just used ice lance, and kept going in a big circle. Never had to stop (expect for some herbs and chests), never ran out of mana (mage armor), and this got done fast.
For Ravenholdt i borrowed a friends rogue (he quit wow, so I payed for his account for a month) and lived in LBRS/UBRS. I couldn't get one rogue on my server to farm them, so you need to have access to a rogue, even if you level one up yourself to 60ish.
For the Shen'dralar, you wan to start looking to the AH as soon as you start working on this achievement. You want to be looking for the books obviously, but start scanning for Pristine Black Diamonds, and good deals on rugged leather, LBS, and runecloth. You won't find the PBD on the AH all the time, or at reasonable prices, so start early.
Before you head into DM to start doing key runs (which i recommend for the profit you can make), you should spend some time in Strath/Scholo and EPL/WPL. You want to have at least a stack or two of the Stitchings/Blood of Heroes/Skin of Shadow before you go to DM.
Once in DM, remember that while the guards have a higher drop rate then other mobs on the key. If a key drops, then make a b-line for the goblin, so you can turn in and reset, but still kill the guards. Not only will you get more blues and greens to DE and sell, but they may also drop a second key. If a second key drops, go directly to the goblin and free him, then go back and get the key before it despawns. Then the next time into DM, you get to go right to the goblin, only stopping to kill the bosses, then you can farm for the key. Once the key is turned in though, you don't have to reset. You can keep killing mobs until another key drops, or you clear the place out, then go reset.
Also, always pick up the Tannin from Upstairs, and find out where
A dusty Tome
are located, as you can sometimes get a
book that sells well
.
Also if you get a Libram, go right to the Library and turn it in, as you can only have one in your inventory at a time, and if a second Libram of .... drops you'll be sad when it goes to waste. Once your goblin rep is done, I recommend going to DM east and west for farming Librams, if not just to get some herbs from the mobs, and for a change of scenery.
The best way to get your DMF rep up is to either be a scribe, or have a scribe friend. I started out herbing myself, which is fine to get started, but once you make a few noble card or two SELL THEM. They sell for anywhere from 500-1300 on my server right now. If you sell yourself a couple of those, you can use that money to buy the herbs and life you need. I had a bank toon that just had all my DMF stuff (lvl 60/70/80 decks) as well as the mages/swords/demons/rogues. Right from the start, keep your eye out for good deals on cards, and keep your bank alt organized. If you have 17 of the Three of Beasts, don't buy anymore.
So if you spend 2000 gold on herbs and and life, and you make three nobles cards out of that you not only make your money back, but you get other cards you need as well. While death adder/lichbloom/icethorn have the best drop rate for the pigment for Snowfall ink, don't' be afraid to buy the cheaper herbs. You can use the ink trader to trade 10 ink of the sea for a snowfall ink. Didn't seem worth it at first, but you make so much Ink of the sea, it doesn't matter that much.
Once you get yourself plenty of money, you can stop posting the nobles cards and start making decks to sell (I would mix it up with the actual greatness cards you get so you can sell a variety). I made a ton of gold selling nobles cards, and the trinkets you get from the cards that let me buy more herbs and Eternal life faster. Now that I'm done with the achievement I'm about 2k less then where I started (the DMF left Monday, so I spent a lot of time/money buying herbs and decks for the low level decks to get my last 2k rep) but I have about 10k worth of stuff listed in the AH, and probably another 5K worth of extra cards I didn't make decks with yet, so it's going to be profitable in the end. Remember you can turn in the swords/mages/rogues/demons anytime, but the big decks can only be turned in during the DMF. Also you can only carry one level 60 trinket at a time, so you'll have to go back and fourth if you have multiple decks.
Don't forget to check the vendor lady next to the guy you are turn the cards into. She always sells herbs/motes/primals that you can buy cheap AND sometimes something nicer like Arctic Furs for 10 gold.
Sorry for the long winded post, but the key to get this fast is to make money while doing this all , AND not be afraid to spend money to make it happen. Just make sure when you turn in your last deck, you do it in the center of Org, and all your guildies are online!

Some quests that give Steamwheedle Cartel reputation, does not lower your Bloodsail Buccaneers standing. If you -like me-, started to do all quest for the Loramaster Achievement before doing Bloodsail->Steamwheedle, then it's good to know, which quests you must not do:
WANTED: Baron Longshore
Southsea Freebooters
and its follow-ups
I just done this two when noticed this and banged my head agains the wall.. If you can complete the Loremaster without these, than it's some free reputation towards your Isane title. Be sure to check every Steamwheedle quest on wowhead.
I will update the list if I find more quests (I list quests that, according to their wowhead page, does not lower your Bloodsail reputation)
Ledger from Tanaris
Root Samples
(Herbalist only quest)
Deepmoss Spider Eggs
Raptor Horns
and its followup
Blueleaf Tubers
Volcanic Activity
Ziz Fizziks
and its follow-ups
Are We There, Yeti?
and its follow-ups

Well one of the biggest debates here is how to handle grinding the goblin rep. Your choices are DM runs, or killing pirates.
As clearly noted by the loot log above, you will make a ton of money doing DM runs, especially if you have an enchanter somewhere to sell the DE'd mats. You will also be getting librams, leather, runecloth and large brilliant shards which you need for the librams/ogre suits, which will also save you money.
If you kill pirates, you are most likely not going to waste time looting them, as they are low level and drop nothing of value anyways. I think someone figured it up to be about 120g if you looted the thousands of pirates you will have to kill, but that will easily triple the amount of time this task will take, and is obviously not worth the hassle.
Some math behind this grind:
If you have very lucky, fast DM runs, you can still only do it 5 times per hour. Assuming you turn in the Ogre Suit quest once each run as well, I would be netting 2,300 rep per hour (human) . Fact of the matter is that you will most likely have to empty your bags at least once during that time, which involves a 10-15 min hike back thru Feralas to the nearest vendor/mailbox. Assuming no stellar drops, I will make at least 200g for my efforts. So 2,300 rep and 200g per hour in Dire Maul North.
Killing pirates. Either outside Ratchet or in Tanaris. You have an unlimited amount of rep you can get per hour. To beat the 2,300 rep per hour, you only need to kill 8 pirates per minute, as the pirates give 5 rep to closest goblin town, and 2 or 3 to the rest. That is EASY to do as an 80. You could go as far to say that you could kill 20+ per minute assuming a steady respawn rate, which would be twice as fast as DM runs. Only trade off being your not going to make any gold doing it.
To each their own when picking which strategy to use. Your going to have to farm DM at some point anyways, but then again it very well may be worth it to super grind back to honored/revered in a few hours and then commence the DM runs later on when you need to.
Lastly, in regards to rogues and pickpocketing. I have quickly discovered that Tyr's Hand is a horrid place to farm these if you are not 70+ . My poor 55 rogue has yet to successfully pickpocket any mob in Tyr's Hand because they are all Elites... No bueno lol

I just finished this Feat tonight, for the long Goblin grind I chose to grind pirates until Ratchet/Gadzeton Revered. One thing that helped break up the monotony for Dire Maul is you can do select quests in Barrens, Winterspring, Desolace for goblin rep without hurting your Bloodsail Rep. But other than that all my goblin rep was Dire Maul turn ins. Mainly the Key. Today I ran Dire Maul for approx 8 hours and out of that had 7-8 key runs with no keys dropping at all :( Takes me about 15 mins to clear DM as prot paladin.
Here's my armory ..
http://www.wowarmory.com/character-sheet.xml?r=Twisting+Nether&n=Clo%C3%BCd
a tip for those farming Ravenholdt, the mobs inside LBRS are excellent source to pickpocket. I had a rogue pre-bc at level 60 so I leveled him to 68 and then began farming lockboxes.
Good luck to those of us who have no lifes to achieve this.. from start to finish took me 10 weeks or so.

To match DM rep gain, you would need to kill 560 mobs per hour if you don't do the quest and 680 mobs per hour if you do the quest. This alone seems like they're equally efficient, but having spent MANY hours farming pirates and venture co. goblins, I know that respawn times start to become noticeable VERY quickly. Also, DM has Libram drops, Darkmoon Faire card drops, and if your character that you're farming this title on is a rogue there are tons of Heavy Junkboxes to be had. Also, by doing DM runs your Cartel rep stays the same for each faction, whereas farming pirates will leave you with Exalted Gadgetzan and 0/3000 neutral with Booty Bay, Ratchet, and Everlook, which is just demoralizing and annoying.
I'm sorry but your whole post is full of misinformation..
I've been grinding pirates for the past couple of days and I can say it is 100% the best option to take. This is my tactic: I am mage using a standard pve fire build, I use mage armour while grinding so with my spec I have 100% mana regen in combat. I havn't tailor made a spec / gearset for grinding or anything, just went out there and grinded.
I farm the pirates just south of ratchet. There are afew reasons for this being the best spot..
1) The respawn rate is incredible, they pretty much spawn as soon as you have killed them, I don't know how people are coming up with low spawn rates but my conclusion is your just doing it wrong.
2) They are level 15 mobs, This is a HUGE factor which i'm suprised no one has mentioned yet. It only takes one icelance to kill them, therefore im constantly moving while killing them. I tried farming in tanaris but having to stop and cast for 3 seconds makes it much slower. I can kill
two
mobs in Ratchet for 6 rep (for all the goblin towns), faster than I can kill
one
mob in tanaris for 6 rep (only with Gadgetzan) so go figure ;-)
3) Finally because of the above reasons the rep gains are far far better than anywhere else. I'm killing 25+ mobs per minute... (one mob per second some of the time, with only 6 seconds downtime to evocate for mana every 3 minutes or so. Maybe it's my gear but I don't need to drink at all) While getting ratchet to revered that was 9000 rep per hour and 4500 for the other goblin towns.
Using your method of calculating it for each faction..
1750 rep * 4 factions = 7000 rep/hour (without Gordok Ogre Suit)
2125 rep * 4 factions = 8500 rep/hour (with Gordok Ogre Suit)
Farming pirates gives:
Before Ratchet revered: 9000 rep*1 (for ratchet) + 4500*3 (for the other three towns) = a whopping 22500 rep/hour (more than three times faster than DM runs...)
Ratchet Revered: 4500*3 = 13500 rep/hour (Still alotttt faster than DM runs, and that is with only 3 rep per kill)
As you can see, even using your perfect drop chance of at least one key every run (killing all the mobs to try and get this would probably take longer than 12 minutes anyway.. and won't give you a 100% drop chance, there is still a chance you don't get any rep for a whole run. But i'll give you the benefit of the doubt.) The pirate farming method is far superior.
Of course you have to take into account the libram drops (I won't even include black diamonds.. the drop chance is so low I havn't even seen one in over 30 runs..), That's why once i'm revered with each faction I'm going to move to DM runs to push up to exalted. So I get the best of both worlds, while doing the grind more than three times faster than I would by farming DM.
Also you don't get exalted with one faction and neutral with the others. Right now im Ratchet revered and friendly with the other three, Considering even getting rep for only three factions is still faster than DM runs, I'll do this untill all 4 are revered and then I'll move to DM runs at which time all four will reach exalted at the same time.
I certainly don't think getting exalted more than three times faster is "demoralizing and annoying" :)

Avast Ye, Admiral!
and then some after a while you can't get more rep time to move on
Second
Kill
Waste Wanderers
out in Tanaris Desert (don't fly take portal from Dalaran to Caverns of time aka C.O.T) Save all
Wastewander Water Pouch
that you find they are turned in once you can which is at Neutral with gadgetzan to
Spigot Operator Luglunket
and do this till i don't get rep from kills anymore which is honored
Third
Kill
Southsea pirates
south of Ratchet till no more rep can be had. at this point pushing till honored with ratched made gadgetzan hit revered do to flow over rep
Forth
Kill Venture Company employees that only reside in
Stranglethorn Vale
or
Grizzly hills
till i can't get anymore rep. (honored)
Fifth
Kill
The Syndicate
till i can not get more rep.( just before revered)
Sixith
then i will have a rogue friend help get
Heavy Junkboxes
for the quest
Junkboxes Needed
these he can pickpocket from and
Gordock
inside Diremaul where inside we can collect the 3 out of 8 Librams you can turn in for rep to rep up with
Shen'dralar
as well as do
Free knot!
and
The Gordok Ogre Suit
as well as Saving and
Darkmoon Faire Cards
as they drop
Finally
As I'm doing all the above steps inbetween raids, I can play my alt that is a incriptionist and make all the
darkmoon faire rep decks
using herbs from chests and drops as well as farming some

I think people are spending far too much time worrying about how they'll handle the Bloodsail Buccaneers/Cartel grinds. To me, this was by far the easiest of all the grinds, and certainly not the most interesting grind. What takes more thought and calculating are the Darkmoon Faire and Shen'dralar reps, especially for people who, like me, have not had these reps up on their characters at all.
From what I've seen and read, it appears as if farming and buying the herbs to create/turn-in the 1440 Rogues Deck's might be the most efficient way to do it. I wonder what others think about this. But if I make a single nobles deck from farming northrend herbs, I can use that gold to buy a lot of lowbie herbs.
For Shen'dralar it's just a lot of AH farming to make sure you pick up the librams and pristine diamonds. I'm getting closer to my title, but I still have at least another month or two before I actually hit it.
Good luck to everyone!

Hi,
Can anyone please confirm that they have used an engineer mailbox inside an instance in DM north to return the Libram Quests for Shen'dralar rep?
I Have seen this mentioned in some places but i have gotten the picture you cant use these indoors.
Can anyone comfirm do the Engineer mailboxes work for this? As this would significantly ease the process of turning in the quest items.

Here is my recommendation for the grind. It took me only about a month and a half from start to finish for the title, and in the long run I ended up spending only 2,000 to 3,000 gold. It also helped that I did my farming between 12 midnight and 6:00 am. I don't know about other servers, but any attempts during the day to run an instance will get you nothing but "Additional instances cannot be launched. Please try again later".

I went for this one first. It was the easiest part of the achievement and it gave me a nifty title to wear while I was farming DM. My friend Earthshaper and I just slaughtered guards for a whole night and within a few hours I was done.
Ravenholdt:
Next on my list was the Ravenholdt faction. The guys at Durnholde keep in Hillsbrad have insanely fast re spawn times, often re spawning in just seconds. If I remember, I got an average of about 3,800 rep an hour killing these guys. This will take you all the way up to 1 point shy of revered. In total, you will spend roughly 5 1/2 to 6 hours running around in circles.
After hitting nearly revered, you will have to start farming heavy junkboxes to raise your rep. Zoraxxi and I spent our time in scenic lower blackrock spire. (He discovered that if you ran all the way to the end and waited 10 or so minutes, you could pickpocket all the same mobs on your way back to reset the instance.) They are 75 reputation per turnin and you get a unique throwing knife as a reward. Consider yourself lucky if you have a travelers mammoth... each of those knives vendors for over 1g.
Luckily for this part I had my own rogue from pre-bc that I could farm junkboxes with. My friend Zoraxxi also helped me on this part on his rogue as well. This part can easily be done in a few days if you have the patience to stare at nothing but orcs and ogres for hours on end.
Shen'dralar / Goblin rep:
Be sure to check the auction house as frequently as possible for the items you will need for the Shen'dralar rep. You will need Pristine Black Diamonds. Large Brilliant Shards, and Librams. I had gold to spare and at the time that I was doing my grind I guess the diamonds were in high demand... I ended up paying nearly 100g each for some of them. Sometimes I got lucky though and found them for 2 g. Just keep at it. Look for cheap librams as well. These are not to fun to farm in DM as they are unique, so if you have money to spare and find a cheap libram buy it out.
The librams require Large Brilliant Shards along with either Blood Of Heroes, Skin of Shadows, or frayed abomination stitchings depending on which libram you have. The blood of heroes (used by the Libram of Rapidity) is by far the most annoying thing on the planet to farm, and if at all possible avoid these like the plague. Skin of shadows (used by the Libram of Focus) drops pretty freqently off the mobs in Scholo and will take you little time to farm. Same goes with farming the frayed abom stitchings (used by the Libram of Protection)from UD Strat.
These librams are unique, so when you do go to turn them in , you will probably want to hire someone to drop a mailbox for you or else your in for a lot of running.
For the goblin rep, you will need to run Dire Maul some 200 or so times to get a shackle key to free Knot and an ogre tanin to make a gordok ogre suit. These require a LOT of rugged leather and bolts of runecloth. You will get plenty of runecloth farming your rep, so don't worry too much about buying those off of the auction house. All you have to do is get a tailor to turn them into bolts. (Zoraxxi, again, I owe you for sitting there for having the patience to deal with all the runecloth I sent you)
To do the free knot quest you will need to find a Gordok Shackle Key that drops mainly off of the named Guards, the Gordok Bushwhacker that spawns when you loot the tanin basket, and sometimes randomly off of other mobs. You can only do 5 runs per hour, and I was faceroll ret spec, so I just killed everything. I had easily finished farming my librams long before I was done with the goblins and I got a key on almost every run.
Darkmoon Faire:
When I originally started my grind, I had read up on a guide from someone who had already farmed the title. The guide told me to farm / buyout low level herbs and make the lesser darkmoon decks and turn those in. DO NOT DO THIS. IT IS A HUGE WASTE OF TIME / GOLD. The card has to summon a darkmoon fortune teller that you have to talk to for your turnins. The fortune teller lasts for some 2.5 minutes but the card has a FIVE minute cooldown.
What I'd recommend doing is camp the auction house. Every time you log on, before you log off, and any spare time you have should be spent looking for cheap Darkmoon cards. I started this insane grind with some 38,000 gold. I am now at 37,000 gold after having sold all the decks that I made from the bought cards. On my server, berserker cards were easily selling for 330g each. Illusion cards sometimes sold at 425g each, and undeath cards were bought out in no time for 1300g each. Most chaos cards are not very expensive either. At any random time you can proably find at least a few cards for around 30-39g each, some maybe even as low as 15g!. Same goes for prisms and undeath too. Even if you were to pay 100g for each undeath card, you will be easily making a 500g profit that you can use to purchase 2 or more chaos decks for rep!
Good luck to all of you who try this feat of strength. It may seem daunting at first but keep at it. This far gave me something to do for the last 2 months AND got me Rivendares Charger ^.^. If I can be of any help feel free to throw me a whisper on Burning Legion horde side.

i only recently found out about this achievement,
after
already getting the Bloodsail Admiral title
and
grinding Booty Bay rep (through silk/dye turn-ins) back up to neutral. since i want this title, i'm on my way to Booty Bay to once again raise my Bloodsail rep from hated to honored...
...insane in the membrane indeed.

Great read, but theres something I dont get.
If i get honored with bloodsail, and then start raising reputation with booty bay, wont my reputation with bloodsail drop?
Have to kill pirates to raise with booty bay and the other goblin cities, so how does one get around that?
Ah, figures the pirates southsea pirates and so on, dont give negative rep with bloodsail

Alright chaps, hit this achievement after 14 days of work FROM SCRATCH. The guides are out there so i shant bore you. But i will offer some tips which are overlooked by most sites.
Overall tip: Other factions auction house. Go.
Shen'dralar-
-Every 40 minutes, relog 1 minute and spam trade channel and auction house for Pristine diamonds. Ask people to dig deep in alts and their banks. If you spam too much you will cause people to think theyre worth too much. people were so bad on my server the diamonds ended up at 300g by the end of my 2 week grind. I never bought for over 100.
-Blood of heroes, kill the mobs after every loot or i think the bloods wont respawn for ages. After every server reset go round as they will all have respawned.
-Find a quel serar book. this gives about 1k rep total. (paladins and warrs only?).
-In 2 weeks of spamming trade there is a good chance of a lucky strike. someone giving up the insane? someone moving server and cant carry their librams over? Theyll remember your name and they will come to you first.
-/who dire maul, whisper, "if you get any diamonds, throw me a tell". Same for strat, scholo etc.
Darkmoon Faire-
This ones a biggie. Have about 10k gold capital. beg/borrow it whatever.
-Be there on the first day SO SO IMPORTANT. i got 30 decks handed in from others on the first day/.
-Have your own trinkets that you give to the other person for their decks. This removes the need for trust. hand in the deck straight after to get your trinket "back".
-if your low on capital, and there is noone left with decks. Go to AH and buy all the cards ASLONG as they amount to LESS than you can sell the deck for. If you can buy undeath cards for a total of 1000 gold (which is insane prices per card) you will still have no problems selling that trinket to someone for 1000. keep rotating this.
-The rogues decks are only useful if the DMF has just left town and you have under 4k rep still to go.
-Contact anyone selling ANYTHING relatred to cards on trade channel. If they have a card/deck/trinket its likely they will have more.
-Focus on Undeath/prisms/chaos. These trinkets still sell well and are most popular in terms of available cards on AH.
Ravenholdt-
-Don't stop moving. you can pickpocket while moving you should never need to stop.
-Work round in a large circle from the right side of the entrance gate of TYRS HAND all the way round until your back to the other side of the entrance.
-If you don't have a rogue, pay a rogue to let you borrow his rogue. I payed a chap to lend me his account when he was at work. I payed about 2k for 2-3 days which was all i needed. a focused rogue can collect all 1400 boxes in a total of 24 hours.
-Shifts do not work well for the initial grind of syndicate mobs, purely because there are so many bumps and ways the sleeping person can dismount is far too annoying.
Goblins-
-To prevent burn out alternate the DM key runs with farming pirates in Ratchet. Do from hated to hostile in Dire Maul North. Then go farm pirates in Ratchet. Farming them is so insanely fast in comparison and you arent going to be missing many librams really. If your doing it with a friend you can work shifts. One plays while one sleeps. dual box with the sleeping person mounted following you. Never let them get stuck.
-(Pirates) Do a large circle starting from 63,47~ (The start of the mobs) work in a moon shape all along the coast, up the ramp, round the cliff and then jump down back to the beginning. they will always respawn by the time your round it. Time yourself to make it more fun. Think my fastest run was 3 minutes?
-In Dire Maul get Loot & reset addon. It tracks when your instances reset to allow you to make the most. Make sure you do all 5 instances ina row so you can use the remaining time to have a break or to farm trash for librams.
-Librams from bosses stay long enough for you to get to the mailbox and back. Librams from trash does not.
-Remember to pick up the ogre tanins from the crate and the pot every time. The bosses have the largest drop rate of keys, make sure you kill them all if needed.
-Save all ogre tanins to the end and make sure you continually buy enough leathers from the auction house that youll have enough by the end. to hand them all in at once.
-If your fast enough you can reach the patrolling 2nd boss before he goes down the ramp. in terms of pure key runs this will save you about 2 mins per run if u can get him before the ramp.
Bloodsail-
-Max out your bloodsail to 11,999 while you can.
A) You'll regret it later (if you don't do it) when you don't have a lovely full green bar on your reputation page.
B) Even if you know you wont want the full bar, you can turn in cloth to the steemweedle cartel for the equivilent of 11,750 "free" reputation. Doing this will save you 22 key runs. In comparison to the extra hour it will take you to grind the extra 12k.
-Group up. each person increases the spawn rate x2 (i think). will go from start to end in about 2 hours flat. Doing on your own will TAKE &*!@ING AGES.(##RESPBREAK##)8##DELIM##Thulm##DELIM##Removed the offending links.

Wanted to add this bit of information with a screenshot:
http://i282.photobucket.com/albums/kk255/aduroris/insane.jpg
"Petition: Insane in the Membrane
Answer: Once you have gained honored with Bloodsail Buccaneers you will gain credit for that part of the achievement, and able to move on to the Steamwheedle faction. Lowering your reputation with Bloodsail will not remove the credit towards the achievement."
While I didn't ask for an official response about timing, I'm going to assume that the following is correct:
To gain credit for any part of the achievement, it had to be active after achievements went live. This means that in order for the game to register that you've been honored with Bloodsail Buccaneers, you had to have been honored at or after achievements came out. If you were honored and you lost your reputation with Bloodsail before achievements came out, I believe you would not get credit for it.
There are achievements that are tracked by tabards and such (C'Thun for example), and I'm not sure if anything like that can be tracked for Bloodsail.

Possibly a stupid question, but does anyone know if you have to have all these reps at the same time?
EG, Exalted with Gadgetzan AND Honored with Bloodsail Buccaneers otherwise you cant get the achievement?

Where can I find the achivement for this particular achievements under the Achivement Tab, so I can track the progress and make sure it registered an honor/exaulted before I work another reputation. I'm not sure if it matters, but my character is a deathknight, which did not have any of the old world achivements.

Though the information is varied from what I've seen, this is my experience with the whole "at the same time" question.
Short version: I'm really insane and have two characters with the title, achieved about 2 months apart. Both of them were exalted with goblins before the title was added to the game. Both of their titles were gained while their goblin faction was still hated. In my experience, you do not need them simultaneously.
Details:
My priest,
Ny,
got her Insane title June 18 or so. When the Insane title was announced, she already had exalted goblins and Shendralar, but I had never done Bloodsail rep or intended to before that. I decided to save Bloodsail for last and started leveling a rogue and buying cards. At that time, I fully expected that I would have to redo all of Steamwheedle Cartel to get my title.
On June 17 I was exalted with all the reps, and hated with Bloodsail: goblin killing time. I got most of the way through that day, then hit honored the next day and was very surprised, but oh so relieved when the achievement popped up. I was sitting at exalted Shendralar, DMF, Ravenholdt, honored with Bloodsail, and fully hated with goblins. My time as exalted with goblins had in fact count toward the achievement.
Now, I'm quite insane, and did not mention my paladin,
Nya
, who I also two-box all of the time. Before June 17 she was exalted with goblins, but nothing else. Come the 18th, she was now honored with Bloodsail, but hated by the goblins, and looking at most of DMF, Ravenholdt, and Shendralar still to go. At the time I figured I got lucky with the priest, but it's going to be a while for the paladin, and I still expected I would have to regrind goblins for her.
On August 23 I turned in my final junkbox and got my paladin's title. She was still hated by the goblins at the time. Then I went to the beach and saw Inglorious Basterds; really didn't wanna look at my computer by that point.
As of this post, their profiles reflect the same reps as when they got their titles.
Shendralar - Exalted
Ravenholdt - Exalted
DMF - Exalted
Bloodsail - 11999 Honored
Goblins - 0 Hated on Ny, about 17000 Hated on Nya. (All from keys/tannin gained during her Shendralar runs)
In all likelihood I'll redo my goblins anyways, at least up to where they won't net me on sight, and especially with the goblins becoming a part of my faction next expansion!
So, on my VERY limited sample size of 2, I'd say if you aren't going to end up with everything at the same time, don't fret, it still worked for me. If you have the chance though, do Bloodsail before goblins though! Don't risk it!
I'll try to submit a screenshot sometime. It's kinda creepy/sad/awesome seeing them together with the titles.

I started with the 'obvious' path to do first and that is grind Bloodsail rep to Honored. I started by myself being prot. Although, it wasn't hard if the guards kept on coming, it was a bit of a pain to kill them so slowly. I then saw a group of 4 other people and asked if I could join theres. By the time I joined theres I was just underneath Hated and I only had to grind for another hour with this group to get to Honored.
The next rep I did was Darkmoon Faire. I'm a JC and BS and although they aren't as profitable as Mining/Herb etc, I do rake in a bit of tips and I can get a big, big profit by playing 'smart'. With the introduction of epic gems I boosted my gold gain every day by HUGE percentage. On the server I play on, King's Amber sell for ~150g, and the Smooth King's Ambers sell for ~250-300g, so I bought 10-20 uncut Ambers and cut them into Smooth ones and sure enough, after studying at Tafe, I came home and saw a big stack of loot in my mailbox with a good profit. I did the repeatable quests to get to Friendly then I bought the Chaos Deck which sells for relatively cheap (300g) seeing as its trinket is 'mediocre'. I repeated this till Exalted, occasionally buying Rogue decks.
The next rep I worked on was Shen'dralar. This was the 2nd easiest in my books. I went into DM North, rounded up everything I could then AoE'd them down. I would collect the 3 different Librams, then make a break for the nearest mailbox and mail it to an alt. This only took me about 4 days of ~5 hours each day to get all 88. After I got the Librams I hit up Strath and doing only 7 min Baron runs I looted more than enough Abom Stitchings in a few sittings. I bought the others via AH. While I was running DM I also did the Free Knot quest which I completed 13 times (horrible RNG for key). When I got the mats I tipped an Engineer 100g to use up their Mailbox CD in DM Library where I handed them in one at a time to the questgiver.
The next rep I worked on was Steamwheedle. I did this with a friend and it took us about 3 weeks of killing Pirates to hit Exalted from 0/36000.
Last but not least was Ravenholdt which in itself is not too difficult but I, not having a Rogue alt, power levelled like I would never have thought. 1-55 in 3 days via Heirloom items and paying for runs and questing like no tomorrow. I farmed Tyr's Hand for a good 12 days to get all the necessary Junkboxes then mail them off to my main where I happily, but very boringly (is that word?), handed them in, but the thought of knowing what my reward at the end was kept me going.
All in all, this is NOT only for the hardcore gamer. I did this achievement in ~6 months and I get home around 5pm and start playing at 6pm and go to bed at 2am (which I raid for 3 hours each night). The best advice I can give for anyone is to pace yourself! Well worth the effort.

DO NOT, I repeat
do not
do Bloodsail first. You will get credit for the achievement even if all your Steamwheedle rep is at hated, provided it's been at Exalted before. Apparently achievements that require rep are toggled on but not off even if your rep changes.
My guildie got it it this morning as soon as his Bloodsail hit Honored.

I tryed this before i got honored with Bloodsail Buccaneers and it showed them in first line under Insane in the Membrane on my screen. After that i got honored with them and than they disappeared from achi tracking, than i killed few pirates and got frendly with them and still there are no Bloodsail Buccaneers on achi tracking. This should mean that u
dont
need to be both honored with pirates and exalted with goblins at the same time.
P.S. dont press Open Achievement on tracking window or ur game will bugg out.

Here are a few things I have learned along the way that you all may find helpful.
1. Farm your junk boxes in DM north. You can get librams, cloth, diamonds, the Ace of Warlords, goblin rep and even leather if you can skin - all in that one instance. Just make sure you have plenty of bag space!
2. Librams: Farm your mats for the book turn ins ahead of time and bring a stack of everything along with you. The books are unique so a quick side trip from DM north or west saves you the agony of leaving a book behind, or worse yet, running to Feathermoon all the time to mail them out. If you farm books in DM West, the best place for the libram drops, you can make it to the turn in and back before a duplicate book drop despawns if you move fast (from most places in DM West and some places in DM North as well)
3. Goblin Rep: The pirate area outside of Ratchet is the easiest to farm. The mobs drop fast and sometimes respawn instantaneously. Baron Longshore, who gives 25/12.5 rep spawns in 3 different places, just not all at once. It is possible, with a little effort, to set the timing on his spawning so as soon as you kill him on one spot he pops up in the next, then the next. After that there is a short respawn period, probably the usual 7 minutes or so, and he will spawn back in the same pattern, as long as you are there to kill him right away.
4. Cards - Start buying them up as soon as you begin this grind, it is the most costly part of this achievement. On my server the primal life prices hit 12 gold EACH making it difficult to purchase mats to make level 70 cards. Getting cards at a reasonable price is possible, but takes time and patience. Some people recommend going after the smaller 3-5 card decks, although on my server this became impossible when a couple of jerks decided to corner the market on low level cards and drive the price through the roof, I am not sure I agree with that strategy anyway. Buying the higher level cards when they are priced right, I usually look to spend 15g for a level 60 card and 20g for a 70, makes selling the trinkets profitable, at least enough so to make your money back. I made about 50 decks and used these to trade the trinkets off for decks again, if you do this when the fair is not running, you can actually charge a little money - then use that money to tip people for allowing you to turn their decks in! Also, as mentioned by another person earlier, I had some success spamming "BUYING ALL level 60-70-80 CARDS!" There are always people who will laugh at your offers because those 4 cards they have been hording for 4 years are CLEARLY worth more than you are offering, but there are also plenty of people willing to help you along when you tell them what you are doing. I even had one guy mail me a bunch of random level 80 cards for free! While you are checking the AH (as much as possible) look for Ink of the Sky at a good price as well. It is used to make level 60 cards.
This is a long and arduous grind. I just hope Blizzard doesn't decide to nerf if like they did with the Wintersaber mount grind. There are not many people willing to undertake these types of grinds, and it's a true slap in the face when they make it so much easier for others who were too lazy to before.
Good luck all, if I think of anything else I'll be sure to post it!

I can confirm that you do
NOT
need reputation at the required levels at the SAME time to gain
Insane in the Membrane
I made exalted with the 4 Steamwheedle Cartel cities back in TBC when I did Dire Maul key runs out of boredom. I then did Darkmoon exalted this week, thus ending up with Bloodsail honored being my last requirement to finish the job.
Here
is a screen shot from when I "dinged" Insane today, and here is a link to my Armory:
http://eu.wowarmory.com/character-reputation.xml?r=Ravencrest&cn=Nelle&gn=Borked
Notice the fully HATED in all of the 4 Steamwheedle Cartel cities -
So the GM stating otherwise was wrong
Now on to make Bloodsail Exalted as well, before I can redo my Steamwheedle Cartel reputation, going to be a very exciting month in Badlands *yawn*

I think ryan is correct.
I had all the factions for this achievement including Steamwheedle at exalted except for Bloodsail which was at 0/36000 hated. When I grinded out for Bloodsail and got honored with Bloodsail, I received the Insane achievement / title immediately, even though the Bloodsail grind had pushed all 4 goblin reps to 0/36000.
In other words, I received the achievement while my Goblin reps were all hated (they had been exalted before). So apparently, they are not a package deal and you are not required to have them all at the same time. I wasn't expecting to get it this way, but hey it was a nice surpise.
But, I would think for ryan's milemarker explanation to work, you would need to "check off" each requirement after the Feat of Strength was introduced. So if you had honored Bloodsail before 3.1 and then lost it, the tracking system would have no way of knowing. Being honored with Bloodsail post 3.1 or being exalted with Steamwheedles post 3.1, and then losing either, I don't believe this would penalize you. Else I wouldnt have the title. But I do.
Doing it this way saves some time, less goblin rep grinding by doing goblin reps first (not so bad) and then Bloodsail (easy). But if you are worried about it counting for you even though it did for me, the safe guaranteed way is still grinding Bloodsail first (easy) and then goblin towns from hated (insane). Glad I didnt have to do it the insane way :p

guys i accidentally found this but it's damn useful for the steamwheedle cartel, in feralas, near the pier where u get the boat to the island there is a goblin, he gives repeatable quests which give 385 rep with each faction, and there's 2 quests, refueling the zapper and zapping the giants (or something along those lines), it's an easy quest and less monotonous than farming lvl 40s and less hard work and cheaper than free knot, it'll give u a grand total of 770 rep a day, which is pretty good, and it only takes about 5 minutes to do it, much faster than farming kills and running around Dire Maul like a headless chicken on a killing rampage

I also had exalted with the Goblins as the achievement was released (counting toward my 40 reps title) and then ground out the buccaneers. I hit exalted with the Shendralar and Ravenholdt on the same day and got my achievement even with goblins being only back into honored (a very long grind from 0/36000 hated in Dire Maul). So to confirm, you only have to have had the reps at some point in time to the required level after the release of the achievement; in a similar fashion to

Hey Guys and gals,
This is my very first wowhead post and i wanted to clear up some math on the goblin rep part of this achievement if you are a player like me and started with 0/36,000 hated with all of you goblin factions and are not interested in the dire maul strategy.
Start with the area of pirates that you like to kill the most. By this i mean the place you want to spend the most time. For me it was the "Lost Rigger Cove".
Kill pirates there until you are at full honored rep. This is a total of 12,600 pirates.
Then Switch to a second desired location and kill pirates there until you are exalted with the first. It's a total of 8, 800 pirates in this second location.
It takes 42,000 kills to get Everlook to exalted so after this point you can kill pirates in your favorite spot until then. I found this method to be helpful because it let me completely avoid killing the venture co. goblins which i found to be too spaced out.
This method does not maximize the amount of rep you will gain with the third city but does get you there at the same point as everlook. It also minimizes travel between zones.
Hope this helps people with the rep grind!

Just about finished this up...and let me tell you, there's no -bad- advice on this page, however I took a little bit of everything and put it together into something streamlined that works for me.
Grabbed some friends that wanted Bloodsail Admiral, aoe fest in Booty Bay, yada yada yada we're Bloodsail Admirals. Took about 2 1/2 hours with 4 of us.
Began solo clearing Dire Maul North (feral druid), killing -everything- from the front towards the back until I'd get a key. I'd go Free Knot. Continue clearing until out of normal ogres, then bosses, or until I got another key. If I got another key out of the same instance, leave, reset, Free Knot, begin clearing all over again for a key and then reset again. Otherwise repeat as if I'd just started. As one of my professions while doing DM was enchanting, I d/e all loots, especially the blues from bosses, for Large Brilliant Shards to use for turning in Librams. I also hoarded all ogre tannins, both from the cache and from Bushwacker's basket (who is also a great drop rate on the key).
Whenever my bags got filled, I would teleport to Moonglade, fly to Darnassus, mail all Librams to an alt, mail all runecloth to my alt with tailoring, and check the AH for any other librams, as well as Pristine Black Diamonds (what usually happened with these is I'd buy them out from 5g up to 20g a pop, then let them come back down again over the next few days). Hearth to Feralas, go back to Dire Maul and start over again.
Once I had a sufficient stock of Librams, Diamonds, and Shards, I would then take a break from DM, proceed to the plaguelands, and ride all over hell and back picking up Blood of Heroes, often offering 10g bribes in general chat for locations to speed up my search. Once I had sufficient Blood to match the librams I had sitting in the mail, I would then reset farm Scholo for Skins of Shadow (anywhere from 0-4 per run), and then do the same with Baron runs in Strat for Stitchings (and yes, I did actually get the Baron's mount while doing this). Return to DM, coerce (bribe) a guildy engineer with a MOLL-E to come to the DM Library, and repeatedly turn in Librams. Had I not run the hell out of ZG in Vanilla WoW, I would have used these for Zandalar rep, but oh well.
My other profession at this time was skinning, which meant I also had an ample supply of Rugged Leather not only from the cache for freeing Knot, but any time I felt like taking out a pack of Hyenas in DM. My tailoring alt bolted all Runecloth gathered from DM, Scholo, and Strat, and I would purchase an appropriate number of Rune Thread and go nuts making/destroying Ogre Suits repeatedly before freeing Knot on my first trip back.
Further benefit of clearing/running DM so much, was also obtaining many lvl 60 Darkmoon cards, -INCLUDING- getting the Ace of Warlords off the King multiple times. This obviously helped facilitate my Darkmoon rep raising. I won't even begin to try to estimate how much time I spent in DM, but once I finished off Shen'dralar rep I switched to the shorter DM clears that result in often meeting the instance reset timer.
I also got 2 Foror's Compendiums, sold both for 1500 each, as well as thousands of gold made from AH'ing lvl 60 enchanting mats from numerous greens that dropped.
Once I finished off the Goblins to Exalted, I leveled a rogue to 65 exclusively for the purpose of farming Heavy Junkboxes in Blackrock Spire. I dropped enough points into Subtlety for Imp Stealth, Imp Stealth Speed, and Imp Sap/Blind. The rest I dropped into combat, and glyphed Pick Pocket to have a 10 yard range. Most of the time I was solo pickpocketing, so I would start in the main room, then do the first part of UBRS (the 7 rooms of orcs that have the glyphs on the floor for opening the door), then work my way through LBRS (skipping the ogres at first, but dropping back down to them after finishing the top floor of LBRS, since it brought me back towards the exit anyways), and by the time I got back to the beginning of Blackrock Spire, all pockets had reset and could be picked again.
On the one night I had a friend helping, we -cleared- the first room on our 80's, stealthed to the Rend event and opened that up, then held the instance while getting back onto our rogues. I would then farm LBRS, while my friend farmed UBRS (his rogue was 70, so less chance of getting caught for him), and eventually the orcs in the first area of UBRS would repop but without re-locking the door, and he'd pickpocket those as well. Whether alone or with my friend, this only took a couple hours a night to pull off until I had the 1400+ junkboxes it takes for a non-human to go from 11999/12000 to 999/1000 (yes, I'm OCD like that).
Currently I'm leveling Herbalism/Inscription, and plan to hopefully have made enough decks to go from 2000/6000 to 999/1000 by the time the Faire comes around in the start of October.

Can you get honored with Bloodsail and then lose it getting the goblins back and still get the achievement?

Can you grind Bloodsail rep first, then just check "at war" and start slaughtering them for goblin rep?

For the Cartel and Dire Maul rep, here is what you do. Don't listen to the other ppl.
1. Get en enchanter.
2. Farm Strath for lots of abom stitchings from the aboms at the end near Baron. Since you have to kill the mini bosses to get the gate to open, disenchant their blues to get prismatic shards (all of the Librams need these). I got around 50 abom stitchings before i left. I also had around 100 or so prismatic shards by then and the Baron mount (since your there, you might as well kill baron and DE his blues plus maybe get the mount).
3. Farm blood of heros from eastern plague lands. This is a #$%^&. All these ppl saying it takes no time at all are wrong. I walked around many times for an hour or more before i saw one and you need two per libram turn in. Farm as many as you can stand (30-40 should be enough).
4. Farm Skin of shadow from the bosses in scholo. Run all the way to the end (need the viewing room key first which you get from the construct in the lower room) and kill just the end mini bosses in the rooms to the left and right of the stairs if your looking down the stairs. Both upstairs and downstairs rooms. The rooms straight ahead of the stairs don't drop them. Don't waste your time with mobs inside the rooms either because they don't drop the skins (you will have to kill the zombie guys to get the end of that room but that's the only one unless you just blindly run into them as your heading to the back). The only mobs that do have a chance to drop skins are the 4 pathing skeletons in the bottom right room looking down from the stairs. Do kill them since they have the best chance to drop the skins. repeat until you have 40-50 skins. DE the blues from the bosses also. This should be enough to last you throughout your libram turn ins.
5. Check the Auctions House for pristine black diamonds. I got them for around 20-45 gold per. You can also get them as a rare drop from the mobs in Dire Maul and you will get some as you progress through this. Buy what you can.
6. Head to Dire Maul north. This will be your home for the next 200 or so runs. NOTE: You can only carry one of each type of libram at a time. This is why you stock up on what you need to turn them in (black diamonds, skin of shadow, blood of hero, etc) Once you enter dire maul, kill every single mob in the instance. If your lucky, you will get 1, 2 or even 3 Libram drops per run. The turn in npc is in dire maul north itself. After you enter the instance, down in the courtyard to the left is a huge door with a long ass hallway, go down there and turn them in. There is a pre-req quest first before you can turn them in so you will have to do that. Some elf located around 68,36 in feralas starts it. its very quick. You have to find a body. Once you do that, you can start turning them in. TURN THEM IN RIGHT WHEN YOU GET THEM. I have waited before since i didn't want to run and the next mob dropped the same libram i had on me already, by the time i ran to turn it in and ran back, the body was gone. I probably lost 5 librams doing that.
7. While you are in Dire Maul. Make sure you free knott! everytime you get a shackle key. Also, loot his chest he leaves behind. if your lucky, you will get an extra Ogre Tannin from that chest. There is also a basket in the far corner after the first ramp upstairs that has a Ogre Tannin in at every time. Just one. Get that too. You can turn these in for extra cartel rep before you free knott. It takes alot of rugged leather and bolts of runcloth fr one turn in so i usually just saved up 20 or 30 tannin before i turned them all in to knott.
8. Repeat until done. You will finish your Shen'dalar rep before you finish the cartel rep since you start -36000 with the cartel and only neutral with shen'dalar.
9. Take a break every now and then.

Here is a sad day for me. After having exalted in everything required besides the Darkmoon Faire, I went to the Pirates Day thing. So a big PvP-fest happened in booty bay, and I turned out joining a raid group. Then I went AFK. I came back 15 minutes later, and I'm now Hostile with all of the Cartel (I was 999/1000 exalted with each of them before this). So yeah, I'm very sad. Note to others out there, don't enter a raid group no matter how fun the idea of killing your opposing faction's players.
So, I now have to basically do most of that grind all over again. =/

One thing i did find out being bored one day doing dailys in Grizzly hills, the venture co ppl on the boat by the riding the red rocket, smokem out quest give 5 BB rep, 3, gadget, 3 ratchet, and 3 everlook, (human dk) per kill. I'm decently geared so i can one -two shot them and have gotten 1k rep in bout 30mins, they also drop anywhere from 7s-25s per kill, along with vendor stuff, and greens.

All classes can be exalted with Ravenholt. If you are NOT a rogue the max you can get is 11999/12000 honored (so you must be at the end of honored). Then you need the help of a rogue to find you lots of :

In addition for darkmoon reputation. I'm very close to exalted now, and my luck I'm both scribe and herbalist. If you have a lot of money and/or good at trading, and talents to tame the AH, then you can just buy all herbs/inks/life/cards/decks and even make money out of it. Your choices of decks are the following:
Low lvl decks: They're the cheapest, but only give 25 rep, so you need a ton of them. A lot of gold and a lot of riding in low lvl areas, and thus you can't fly, you are much more slower.
Lvl 60 decks: they qould be pretty nice, because they do not need any life element to craft, but they need 5 ink (5 pigment) and you just can't farm them fast enogh.
Lvl 70 decks: Oh yes, you need only 3 uncommon inks (3 pigment), you can farm the herbs by flíing, and there are a ton of them, or just buy them in the AH for cheap. You will have a LOT of inks. Rally, believe me, a lot. Seriously. BUT.
You need 3 primal life as well, and for primal life...it's really a pain to farm. You probably want to buy them in the AH, but you just never can get as many, as many inks you have.
Lvl 80 decks: 6 inks (12 pigments) and 3 eternal life to craft just one, but you can buy a lot of them, and you can farm them pretty easy. Just get to WG when your faction have it, and farm some eternal, or make camp in Sholazar Basin/Icecrown and Storm Peaks and farm a lot of herbs. Anyway you'll have to farm a lot, but after all, you can sell the decks (Death and Greatness) for gold (whick from you'll buy moar herbs and life).
As you can see, there are a lot of choices, but anyway you'll have to farm a lot. I would advise to not settle down at one, but do all of them. FArm herbs/life, create cards, stalk AH for other cards, pay 50 to swith card to deck, so use every possible way to attain more decks and thus reputation. I, myself just made about 20 nobles deck this far (lost count at 13), and there were months when I crafted five or six of them. But I still have more than 200 cards, and none of them enough for a deck, and still got that 14 berserker cards...maybe the lvl 1 newbies would be happy to get some of them ^^

I can confirm this, I was exalted with steemwheedle back in BC and was affraid of doing the bloodsail admiral because i'd lose all that rep. But for the insane title it was worth losing and as soon as I hit honored with bloodsail I got the title.

Because it's not clear at all if you need them at the same time, or just have them at one point, here is a response from a GM today:
Issue Reported:I, along with many others, are confused currently with the "Insane in the membrane" achievement requirements.
There is some debate regarding this particular achievement - The intended function of this achievement was to be awarded once all requirements were met, but they were not necessarily supposed to be completed simultaneously. This particular achievement is in review with our developers now and more information should be available at the resources listed below once the investigation is complete. Thank you for your time and patience, my friend.
We are aware of the situation you reported, and we are currently working on a solution. Please check the World of Warcraft website (
http://www.worldofwarcraft.com
) for future updates and corrections. We appreciate your patience.
It appears that it would be best to err on he side of caution.

Can anyone provide a armory link to prove, that this is possible? If it is, will save tons of time, since a lot of players are half revered with cartel from leveling quests.

I can confirm, at whoever was asking, that
you don't need
to have both goblin/BB rep up to honored/exalted at the same time(atm).
You had, until one of the last patches. This achievement is now working like all the other achievements, if you was honored with the pirates then you are done with them. To make it more clear, you don't need to get honored with them, again, after you got exalted with the goblins.
Confirmation:

http://eu.wowarmory.com/character-reputation.xml?r=Arathor&cn=Yat%C3%A1&gn=Natures+Wrath
Bloodsail Buccaneers (-41935) 65/36000 Hated
He also told me that he didn't had to rebuild his rep with the pirates, again, WHILE i actually had to do that( I was exalted with the goblins, actually, and i had to burn the reputations of these 4 factions down and, after i got honored with the pirates, i went farming goblins rep in DiM, again, since i didn't get the achievement).
When i did it, right after the achievement got released(its not me in the movie) ->

After 3.1:

(right from wowhead screenshoots. So now, probably, you just need all the faction listed only 1 time at exalted/honored)

Ok guys, so many different views on this matter of steam rep vs the pirates.
Some of you say you need the pirates at honored at the same time as steam at exalted, some of you dont.
I asked a gm today (091030) and he "thinks" you should be able to do it even if your rep with the pirates isnt at
honored. Gee some help from that guy.
Here is a screen of some of the conversation:
http://i58.photobucket.com/albums/g256/starions/div/insane.jpg
Told him about a video i saw on the tube about a guy who didnt get the achievement when he had the pirates below honored. He asked if it was new. As you can see the gm "thinks" this has been changed in a patch.
Is it just me who is confused about all this and dont wanna take the chanse?

I'm doing this on my main right now. I'm 75% in.
My experience is that when you are grinding goblin rep, simply re-run DM and only kill the bosses. You get a much higher chance at getting a key and it is very fast. You can also easily run back and get a key if you happen to have one, so here is how I do it:
First and most important. If at any time you have a Gordok Shackle Key and you have already turned one in, simply log out for 5 minutes or so and you will be at the front entrance. You are not here to farm anything but Booty Bay rep (other goblin rep is typically higher than Booty Bay, so you can set Booty Bay to your rep bar and monitor it.)
My logic for this is simple. I'm not trying to get librams yet. I'm going for only Booty Bay rep. Librams sometimes drop anyway and I mail them off when I get them because I power-leveled engineering for my own mailbox. Sometimes I get Pristine Black Diamonds.
Zone into Dire Maul North.
Run full speed at the first boss, Guard Mol'dar. Pull him right up the ramp behind him and tank/spank him at the top.
Run full speed from the top your left, skipping all trash on the way to the middle of the circle where Guard Fengus pats to, and where the Gordok Courtyard Key. Kill him right there to avoid adds.
Run through to the courtyard door and avoid all trash on your way to Knot. By now you should have a key, or you may have had one upon zoning in. If you don't have a key wait in the corner where Knot is. Kill Guard Slip'kik in the corner so he doesn't pull adds.
Skip all trash on your way to the Ogre Tannin Basket. Stand in the corner and open it, then wait by the fire for the Gordok Bushwacker to scream at you and run into the middle of that area. Kill him by himself or with the trash that is blocking the ramp up top.
Optional: you can skip Captain Kromcrush or you can kill him. I have found that he seems to have a much lower chance at a key than the other guards seem to and getting to him is slower than getting to the other guards.
Do not bother killing King Gordok unless you are really low on Large Brilliant Shards.
If at any time you get a key and have turned a key in, log out. Don't waste your time on trash -- it's slow and it has very little chance at dropping the keys you need.
I will run DM 5 times per hour and use the rest of the time to read Reddit.com. Sometimes I take an hour and do Ravenholdt rep farming in LBRS. I went to Revered Booty Bay from 0/36000 in about a week casually doing it this way.
The bosses of DM North have a chance at dropping keys programmed into them. No other mob has that chance in the same way.
The keys are green items, and when wow rolls on loot, there is a chance to trigger a green drop. The guards have a much higher chance at getting on that table and very often you'll get either a libram or a key or a boe green. Most trash is maybe 5% for a green table roll, and keys are not as heavily weighted.
Because guards typically have keys!

Look, i just finished mine the other day... this !@#$ is silly simple.
You run dire maul north to get your cartel rep up, and Shen'dralar rep up. While you are there, turn in your keys and librams as you get them. This means you will need lots of libram and ogre suit mats. Stock pile on them and you are good to go. It takes no time at all.
People who say you should kill pirates are just morons. Don't listen to them. It's much easier, safer and makes you more money to do all 5 of those reps (the 4 cartel and Shen'dralar) at the same time.
Go to Tyres hand for lock boxes. NOT UBRS. Again, ppl who say UBRS are dumb. The only bad part is the Darkmoon faire. Its much better to just farm money and buy cards as they come on the AH then it is to farm herbs and make the cards yourself.
I think this achievement is too easy. A dedicated person could finish this in a week if not for the Darkmoon cards and the fact that the faire only comes around once a week at the first of the month.

I would advise starting with Bloodsail rep because of the terrible terrible damage it does to your Steamwheedle cartel rep. In most cases you will end the grind at 0/36,000 so no matter how many more guards you kill you can not lose rep. Continue killing guards until you reach 11,999/12,000 with the Bloodsail. The reason for this is you can then do
Traitor to the Bloodsail
roughly 23 times to quickly boost your goblin rep back up.
If done correctly you will end up with the following rep
Bloodsail Buccaneers
- 499 / 12,000
Booty Bay
- 11,500 / 36,000
Everlook
- 5750 / 36,000
Gadgetzan
- 5750 / 36,000
Ratchet
- 5750 / 36,000
Happy insanity grinding =)

A few points for people.
YOU DO NOT NEED ALL THE REPS AT THE SAME TIME!
I have had sc exalted since wotlk and didnt bother with this because I did not want to trash the faction. It does not matter. As long as you have had SC exalted at some time you are good.
If you decide to do this, plan it early. You will have to start playing the ah in earnest to gather the librams, diamonds and cards needed to grind this out. Plan 3 months if you want to keep a decent gold level.
Go Engineering for one of your professions. The mailbox is a must and the teleporters dont hurt either.
Get a Traverler's Tundra Mamoth. Vending is a good thing.
DKs, exploit em. Grind a dk up to 68 and max out his professions, it takes no time and its handy. Mine is a alc(elixir master, thats why 68)/Inscription
Make a rogue, deck him out in all the hierloom gear, get him to 51, move him into av till 65. At 65 start PP tyrs hand. 20 boxes a day for 3 months is the easy way. Do not forget the PP glyph.
Park the toon that is going to do this ach in epl when u go to PP. There are about 6 spawnpoints for Blood from LHC to Tyr's you will want to gather as much as them as you can.
LOOT EVERYTHING.
You will read about a bunch of people that say its faster not to loot the mobs. This is garbage. You will need the money for Librams, Cards and Diamonds. People will wise up to the fact that someone is working this ach and start to raise prices. IE Libram of Rapidity on my server went from 10g at the start to 250 by the end of the grind. YOU WILL BUY THEM! Dont kid yourself.
Rachet mobs drop:
Savory deviate delights Recipe, you should get at least one a day which is an easy 500g per.
Small Pearls, 100 a stack.
Linen 1-3 g a stack.
Tanaris
Mageweave 8-20g a stack
Green weapons = Greater Nether Essences = 13-20g a per
Wirts Third Leg = about 50-75g per
Make a bank toon with a guild bank. Use him as a clearing house. You will need a whole tab for cards!
Tired of grinding? Farm Blood of Heroes. This was the most tedious of the gathering as its just riding in circles. I used gathermate to assist in this.
Track the ach with this macro /script AddTrackedAchievement(2336)
There are other tips and tricks, but these were the important ones. Remember you are going to be hitting the ah alot so use everything you have to make money.

I decided this achievement would be a challenge.
It has been fun so far, been working on it in my spare time since September 26th.
The one thing I can say is this. Killing pirates for rep would be a *huge* waste of time on Kul Tiras.
The Librams and Pristine Black Diamonds are very difficult to find
at any price
on the AH.
I got all my librams, except for 7 by the time I was through running the Free Knot quest.
I've been buying Pristine Black diamonds any time I see them from 1g to 200g on the AH.
If it scares you to pay 200g for an item you will need 82 of, quit now.
I'm done with Bloodsail, done with Steamwheedle, I have all my lockboxes for Ravenholdt,
82 librams, 60 Diamonds and all the mats to turn in said librams.
I figure I might be half done. The other diamonds and Darkmoon Faire are going to be tough.

Make sure to have a level 1+ alt on the opposite faction of yours with at least 1000g at all times, make sure you do a several checks on their auction house, this was somethign I found very useful.
Here is a comparison of the prices of the items during my Insane grind on both the auction houses
Librams:
Alliance: 200-500g
Horde: 7-200g ( And it wasn't just once I saw it for 7g, and the most common price was 100g )
Pristine Black Diamond
Alliance: 100-500g
Horde: 50-250g
Make sure you make a /who on Stratholme, LBRS, UBRS, Scholomance, and then /whisper people throughout the day and ask if they happen to have any of those and make your offer to them, and ofcourse trade channel works.
Darkmoon decks:
I didn't get to buy my decks on the opposite faction, but I know the prices are very different.. and what I did in the end was that I got in contact with a big seller of all Darkmoon Decks, and I made a deal with him to let me turn all his decks into the vendor and I would pay him 100g , and in the end he didn't even wanted the payment and this person made alot of decks for me, which I turned in and he got the rep in return.. ( Do not expect this to happen on all server, or for you ).
Good luck.

I think it might be worth mentioning that there is a formula that can help you determine how much spillover rep you have to farm with each steamwheedle cartel sub-factions after hitting revered with all of them, incase you changed farming zones before (like I did to not to get bored from farming only ratchet mobs), and don't want to waste rep points. It's something like this:rep=(21000+CurrentPlaceRep-OtherTown1Rep-OtherTown2Rep)/2
Say you got 1000 ratchet rep, 3000 booty bay rep and 5000 gadgetzan rep and wonder whether you have to hunt more ratchet mobs, so you just put the numbers in(21000+1000-3000-5000)/2=7000 rep or 7000/2.5=2800 kills
Do it for each of the 3 farmable factions to find out how many rep or kills (by dividing rep by 2.5) you have to farm in each of the places.
If you get a negative number it means that you overfarmed this place and unfortunately will end up wasting time on extra kills. If it happens to be the place you're farming now, you don't need to do more maths. Just move to any of the two other places you have until you get exalted with the other one, than move to the place you just got exalted with, until you get exalted with all.

For those that haven't yet started to go for here is my tips:
1) You NEED devotion. Forget about dailies badges and other stuff. You will have to focus on this.
2) The hardest part for the achievement has a name and it is called "Pristine Black Diamond". All other things are farmable except Pristine Black Diamonds.
3) You will need a 50+ rogue (60+ recomended)
4) You will need an alt with inscription and herbalism (the rogue can do the job)
5) The hardest reps are Shen'dralar (by far the hardest) and Darkmoon Faire (due to decs needed but way easier than Shen'dralar)
GOOD LUCK!

For the Steamweedle-Bloodsail grind here is the biggest tip I can give...
After you are at
0/36,000 Hated
(aka bottomed out) with Booty Bay, keep killing Booty Bay Bruisers until they stop giving you Bloodsail rep at 11,999 honored. That overshoots
Insane in the Membrane
by just shy of 12k rep. Then kill
Jazzrik in Badlands
once to ding
revered
. This is to prep yourself for the long grind back up. You won't want to. You'll be sick of killing Bruisers and it will be painful to continue. But this pain will save you
worse
pain later.
Since you only need the bottom of honored, not revered you can
trade in 12k worth of runecloth to
Everlook
.
You will lose 500 Bloodsail rep from each cloth turn in. You'll need 24 turn ins of
Making Amends
But it's
WORTH IT
. It's much easier getting12k rep by killing bruisers then turning around and losing it than it is to do Dire Maul runs or kill the various pirates. Everlook will be your last and hardest one to bring back up since nothing you can kill gives bonus Everlook rep.

I found that Pilgrim week is an excellent time to do turn-ins. The Spirit of Giving buff will grant 10% to rep gains, and it DOES stack with the human racial. For Libram turn-ins I'm getting 600 rep this week, and Freeing Knot awards 420 rep.

I just submitted an in-game ticket with a GM on the issue of whether or not turning in cloth with the goblin cities affects the achievement if you've already gotten Honored with Bloodsail. The result - turning in cloth does NOT AFFECT the achievement if you have honored. You just need to achieve honored than you can destroy the bloodsail rep. So hopefully with will save a lot of time.

You might want to get the crusader title from the Argent Tournament before doing this grind because it allows you to buy
https://www.wowhead.com/wotlk/item=46874
the Crusader's tabard. The tabard allows you to hearth to the Tournament and does not share a bind nor CD with your hearthstone. This means you can set your hearthstone to places like Light's Hope Chapel, Feathermoon Stronghold, or Camp Mojache for some nice mobility and when you want to go to Dalaran, you can just use your tabard to hearth to Northrend and fly south.

Today I explored the question: How long will it take to farm enough herbs to reach exalted with Darkmoon Faire via rogues decks?
Answer: About 240 hours.
Givens:
1. To build a rogues deck you need to learn the ability with the inscription tradeskill.
2. It requires 3 cards: Ace, Two, and Three of Rogues.
3. Crafting a card requires 1 light parchment (unlimited supply from a vendor), 1 midnight ink (crafted by an inscriber), 1 hunter's ink (crafted by an inscriber)
4. Midnight ink and hunter's ink are both crafted by milling 1 of 5 specific herbs: briarthorn, swiftthistle, bruiseweed, mageroyal, and stranglekelp.
5. The limiting factor in crafting a card is hunter's ink. There is a 50% chance you get 1 verdant pigment from a mill of one of those 5 herbs. 1 verdant pigment is crafted into 1 hunter's ink. So on average you require 10 herbs to create 1 card.
6. The card created is random. Even if it seems you go on a long drought where you are missing 1 card, it truly is random.
7. You first should exhaust rep gains by the turnin quests which stop partway through friendly. After that, you need 1480 rogues decks (at +25 rep each) to reach exalted.
8. To create 1480 decks at 3 cards each = 4440 cards. Remember it takes 10 herbs, on average, to make 1 card. So 10 herbs X 4440 cards = 44,400 herbs. At 20 herbs per stack, we need 2220 stacks of herbs.
Knowing that, I farmed for 1 hour around Silverpine Forest and Hillsbrad Foothills for the 5 types of herbs. In that time, I acquired 9 stacks. At that rate, it will take about 247 hours to reach exalted with Darkmoon Faire. If I spent 20 hours a week doing this (equivalent to a part-time job), it would still take 3 months.
If I sold my 9 stacks for 18g each, I'd make 162g. Purchasing all 2220 stacks from the AH at an average cost of 18g, it would cost about 40k gold.
The long story short is that you would have to be insane to actually farm the herbs to reach exalted via Rogues decks. There are better ways.

For anyone wanting to do this achievement but doesn't have a level 50+ Rogue yet, and doesn't want to level one any more than they have to...
I leveled my Rogue to 51 and just did cycles around the Arena and surrounding rooms. I started in the room just below the arena, go from the entrance through the locked door (that can be picked) and up the first set up ramps. There is a room with a couple wandering fire elementals and about 5 groups of Anvilrage mobs. From there I want halfway around the arena to the other side, doing the spectator groups along the way. The out of the arena to the hallway before the bar. You can get at least 3 groups of mobs here, sometimes 4. (The group to the left usually has a dog in it, but sometimes it's all humans.) After that I go around the other side of the arena back to the start. There are ~80 mobs in the cycle, give or take a few depending on whether a elemental or a human spawns in some spots.
It took me ~6 minutes per cycle of pickpocketing. At the end of the circuit you have a minute or 2 to organize your bags, and delete the junk (like Pipes) before the pockets respawn. You could also go down to the lowest level before the arena (just after the locked doorway you picked) and get about 5 more groups of Anvilrage mobs if you want to spend the extra time that way.
Note: The Anvilrage groups (before the arena) have only about half the chance to drop Heavy Junkboxes that the arena mobs and after have.
I was lucky enough to have the new Thanksgiving holiday buff (10% rep increase) for somewhere around 100 turnins so my totals below are a bit skewed by that.
I ran the circuit 185 times for a total of 1390 junkboxes. Which is abuot 7.5 junkboxes per cycle. The most I ever got in 1 cycle was 17, the least was 2.
Also, 1 or 2 of the groups in the arena will usually have a Senator of level 54. At level 51 these had about a 15% chance of catching me pickpocket. And they also see you from farther away, so be careful with them. Typically I would try to PP them, if they see me I vanish and just skip them until next time. Other mobs I would vanish and then PP again without worry. There is also a level 54 roaming fire elemental in the hallway after the arena that will see you from fairly far away.
Hope this helps anyone else who wants the achievement but hates leveling alts like me.

Im currently 14k rep with Darkmoon Faire away from getting the insane title. And I've currently spend roughly 2½ months or 250-300 hours played to get this far. I did the achievement in the following order:
* First of all I did the bloodsail rep. Took me only a couple of hours, grouping up with a pala friend of mine. Be aware that this is the most easy part of the achievement - and compared to the other factions, even absurdly easy.
* When you hit honored with bloodsail, you would want to raise your reputation with the Steamwheedle Cartel - and if anyone should be in doubt whether they should grind the pirates for rep or do DM, let me make this crystal clear: YOU REALLY WANT TO DO DIRE MAUL, AND NOT GRIND PIRATES! Why? Because you get librams and pristine black diamonds while grinding DM. Lets say you grinded pirates and got exalted with steamwheedle factions - YES it was way faster than doing DM - BUT now you have to spend exactly the same amount of time in DM to get librams, that it would have taken, to raise steamwheedle from hated to exalted in Dire Maul.
When I was fully exalted with steamwheedle after doing DM roughly 200-250 times, I had ALL the 80 librams and ALL the diamonds I needed to get exalted with Shen'dralar (most diamonds was bought on AH - I only had like 20 from all the DM runs). So dont be a moron and waste your time grinding pirates, do DM!.
* Shen'dralar is fairly easy. Just do DM til exalted with steamwheedle factions, and you'll most likely have the librams for Shen'dralar. Be aware that turning in the librams require you to grind several hours for other mats like stichings from strat, skins from Scholo and Blood of Heroes from the plaguelands.
* Okay, now you want to raise your reputation with Ravenholdt. This is very simple indeed. Grind mobs til revered, and then grind lockboxes til exalted. I spend around 20-25 hours to get all the lockboxes I needed.
* Darkmoon Faire, is argueably the easiest faction to raise, however that is if you got !@#$ loads of gold. By the time I started on my darkmoon faire rep as the last faction, I had only spend 1 month. Now almost 2 months later, im not done with DMF yet. The way im doing this, is just buying every epic deck on AH (Yes im RICH!) think I've spend 25k on decks and cards so far.

ATTENTION! ATTENTION!
Right now during the Feast of Winter Veil is the time to get your Steamwheedle Cartel rep back! Most of the quests for the Smokywoods Pasture grant Steamwheedle Cartel rep, all four pieces of it. Even better, they DO NOT lower your Bloodsail Buccanners rep. This is the perfect time to save yourself a few DM runs!
Happy farming!
-Raw
Edit: The Dire Maul runs can't be done any more as of Cata. :( I'll leave the comment intact to preserve a little bit of Insane lore.

to be honest i find the hardest part to be the part where you get 84 pristine black diamonds. these diamonds are realy rare since no one does scholo/strat anymore, and i see people selling those for 500 gold per one. and since they got such a low drop rate i dont think i can consider them to be farmable.
does anyone have any idea how to easly(well...) get those?

Just a confirmation for players who are looking to begin the Reputation Insanity...
You DO NOT need to be Honored with the Bloodsail Buccaneers at the same time as being Exalted with the Steamwheedle Cartel.
I confirmed this by using the script code to track achievement progress with "Insane in the Membrane" ( /script AddTrackedAchievement(2336) ) on a Level-1 alt and comparing this with my Level-80 main who was Honored with the Bloodsail Buccaneers. As was expected, the Level-1 alt still needed the Honored w/ Bloodsail portion and my Main did not. I then did a few cloth turnins in Winterspring to get my Reputation below Honored. When I did this, Honored w/ Bloodsail portion didn't reappear--meaning I still have that portion completed.
What's the importance of this? Well, instead of killing something like 20k pirates to regain your Steamwheedle Cartel reputation while not touching your Bloodsail Reputation, you can do cloth turnins to regain Neutral status (around 1.5k gold in cloth). This should speed things up somewhat. Also, if you're playing a DK or happened to not have done quests with the goblin factions, you can now, after getting back to neutral w/ the goblins, do low level quests to get to around revered with the Steamwheedle Cartel (since you don't have to worry about your Bloodsail Reputation). This method is not for perfectionist players but if you're short on time, this can make the achievement more attainable to some.

Hey,
I've been farming for this achievement for a bit (honored with Bloodsail, working on Steamwheedle), and I'm wondering, is this achievement going to be removed in Cataclysm? Because the Goblins are joining the Horde, and their changing the "Classic" landscape a lot. Also, I didn't really get what other people were saying, in DM do the Librams drop wholly, or do you need to get someone to make it for you.
Appreciate the help

I haven't seen this posted anywhere yet, but I just discovered a way to get 1500 more Shen'dralar rep if you're a blacksmith. I had both halves of Advanced Armorsmithing Volume 1, so I decided to combine them. You turn the book in to the Lorekeeper in the Athenaeum 550 rep for my human warrior... Volume 2, off the AH cheap, for another 550, and looking for volume 3 now. As Pristine Black Diamonds average 200-300g on my server, I am really excited about saving myself part of the headache, and having three extra librams to sell. Cheers!

I am now insane . lycanthrope on eonar. I thought I'd post some random thoughts.
The meat and potatoes of this achievement is the shen'dralar rep, its so encompassing and will take a bulk of your time completing. It's definately worth it to do shendralar rep and the goblins at the same time in diremaul north. by the time I had freed knot to gain exalted with the goblins I must have found about 60 books, the rest I bought in the auction house.
Thru all that grinding high level mobs I may have found MAYBE 5 pristine black diamonds, the rest I bought. Just to give you an idea of how rare they are.
keep enough mats to do 10 of each book turn ins, its nice when you run out of the mats to break up your DM grind and head to the plaguelands.
learn about the droprate of the gordok shackle key, and really maximize your droprate. I got so good and fast at key turnins that I easily hit the instance max / hour. at that point I would take time to get lockboxes and darkmoon rep.
it took about 4 darkmoon fairs to get exalted selling decks, and working the auction house and selling decks on alliance AND horde side I actually made TON OF MONEY . so much that this whole grind was a big moneymaker at the end.
it was invaluable to have : travelers tundra mount - selling junk repairing. argent squire for mailbox, bank. also by a ton of waterwalking elixirs so when you set your hearth to feathermoon. you can mount across water instead of waiting for the accursed boat.
To get exalted with the goblins by killing pirates, you would have to be INSANE!
well, thats it I wish you luck !

I'm rather confused concerning steemwheedle + booty bay rep, I ticketed a GM just to clarify what's been said already, and he did say I supposedly need both reputations at once. So I then toggled the script that has been linked previously, to see if I had done the bloodsail part, and it said that I still needed to aquire the rep - I did it in TBC. I'm guessing this does mean I'll have to do both bloodsail + steemwheedle once again - or have any of you had the same problem and still managed to get this achievement without re-doing Bloodsail Buccaneers + Steemwheedle farm?

i asked a GM about this issue, and it was confirmed that you no longer need to keep your reputation with bloodsail buccaners aswell as being exalted with all goblin factions at the same time. so you can do the hand ins with cloth or what it is which goes quick.

Anyone knows for sure and is 100 % sure if you get the achievement if you get exalted with the Steemwheedle cartel (Booty Bay, Everlook, Ratchet, Everlook) first then you start your rep with Bloodsail Bucaneers, which makes you lose your rep with steemwheedle?
Does this count?

I am nearly finished (3000 Ravenholdt Reputation left) with this achievement and want to share some of my experience.
First look at this pages:
http://files.spectralcoding.com/files/wow/index.html
http://www.wowwiki.com/Insane_in_the_Membrane
You need Moll-E or the Argent Squire with the upgrade (to get the squire you have to do the dailys every day for 27 days, do it) without it is the hell. Both are better but not necessary (i got only the squire). Reason see below.
Get a Mail addon to autoloot the Mailbox without it you got insane before you really started.
Now first about the AH:
Get auctioneer! It is a really good addon. At the beginning a bit complicated but it is a time and money saver. You will thank the creator when you sell the mats you farmed and when you buy the huge amount of mats.
If you really want this achievement you have to check the AH after you start WoW and bevor you exit WoW
Reasons:
- Librams (Focus/Protection/Rapidity): Not really often in the AH because the librams are only dropping in DM. If you see one buy them! Expect every prize from 15 Gold to 250 Gold. When i finished with Shendralar i got some Librams. Protection and Focus sold for 180 Gold. Rapidity for 50 Gold. Believe me you will need them.
- Pristine Black Diamond: Same as Librams! When i was finished with Shendralar i sold some for 180 G. Pay every prize when you think it is OK, because they are not really farmable and you need 82-84 Diamonds as nonhuman!
- Decks/Single Cards (Rogues, Mages, Warlord, Prism...) for Darkmoon Reputation
- Pigment and Ink to create Decks and Cards
- Sell the mats you got while you earned when you where farming
- mote of life, eternal life.... for the decks
If you use the AH well money shouldnt be your problem if start with an amount of ~ 20k Gold
Create a char on the other faction and check the AH of the opposite faction, too! I got a lot of my librams and Diamonds and some cards and decks from the other AH!
Funny sidenote! When i started with farming Darkmoon Reputation the rare cards were really cheap but after a few weeks some people noticed that they could sell the cards and the prizes were rising. When i was nearly finished they were really expensive. After i got my Darkmoon Reputation the prizes went down!
Bloodsail:
Group and farm until 11999/12000 honored then turn in the Bloodsail Admiral Quest to get Revered. If you finish with something like 1100/12000 honored you will always think: &*!@, i should have farmed the rest of the reputation, but you cant because you already start farming Steamwheedle Reputation
Darkmoon:
Depends on your money, prizes in your AH, how many people are going for the achievement (on your server, of course) and how much do you like to herbalism-farming!
What i did: Set yourself a limit how much money you want to spend for every reputation point. Dont forget you have to pay Librams and Diamonds, too. I expect to pay 20k money for the achievement what was nearly my complete money.
Example: I said to myself at the beginning i want to spend around 15 G (later i went up to 30g) for the small Decks (Rogues, Mages etc.). This is including as an example rogue cards for up to 5g and as mage cards for up 4g! The big decks depends on the fact how good you could sell the trinkets in the AH. If you got a nobles Deck for 3k and you could sell the trinket for 3,5k you get money! But a vanilla deck for 300g is maybe only selling for 150G any you loose 150g.
Herbs: On my server i couldnt afford the herbs for the cards (i just bought the cards and the pigment!). The reason is quite simple. On my server was an inscription guy who was making a huge amount of money with the glyphs but he was selling the green ink in the AH. So i made a deal that he was selling every green ink to me! So i only had to farm buy the common pigment for the cards! Later he realized he could make even more money when he create the cards and sell them in the AH... (luckly i was nearly finished at this point)
Steamwheedle:
Farming pirates is complete bull!@#$ EXCEPT you got a huge amount of librams through other players or the AH! But i am pretty sure you wont!
Reason:
Against the theory only to farm the bosses in dm. Farm the ogres too because you need the librams, they drop also the key and you only have 5 ids per hour! The best way is you have the key of the run before. Then kill the Ogres find a new key, free Knot and loot the new Key.
Pull as much as possible but not too much because the noncaster ogres before the door stun and silence and as a caster class you are really &*!@ed up if you pull too much. Use line of sight to pull also caster mobs (they are no harm to you and die pretty quickly.
I cleared mostly of the ogres before the door (not all) and every ogre after the door until Captain Kromcrush. With this strategy i only left the id a few times without freeing Knot.
If you have enough junk for a vendor and you dont have the mount go to the libary there is a vendor where you can also repair.
If you have enough junk for the AH use Moll-E or the squire.
If you are deep in the instance (as an example you are at Captain Kromcrush) and no key dropped on your way DONT WALK BACK to the entrance! Log out and check the AH or/and farm junkboxes and/or herbs! Junkboxes NOT in LBRS because so you waste one of your instance-ids! They are coupled with all your chars on your Server/Account. You only have five! After 5-6 minutes log back to your main, leave dm, reset and farm again. This just save you 2-3 minutes per run but you have to do ~ 220 runs
So i got an average reputation output of ~ 1500 reputation per hour (without costume) + farming herbs/boxes
I farmed Steamwheedle Reputation only this way and at the end i sold 10 Librams in the AH but bought a lot of Librams before in the AH. So you should prefer this way.
Shendralar:
If you dont have an squire or moll-e and a libram drops you have to go to the libary because if another drops you cannot loot it (believe me nothing is more frustrating). So get the squire.
Most of the people think the Librams of Rapidity is complete worthless because farming Blood of Heroes is complicated. This is nonesense.
Look at the wowhead section for Blood of Heroes. There are two, one for the looted Blood and one for the Blood on the ground. Look at the comments. Get the coordinates of the Blood, combine it with TomTom or gatherer and voila you can easily farm blood. You get about 8-16 per hour.
Skin of Shadow drops also of the mobs in the Darkmaster Gandling room and dont forget the darkmaster. He has the chance to drop one of the vanilla aces!
Ravenholdt:
When you hit revered there is only one option: LBRS! It is easily doable with a rouge lvl 65. There are enough mobs and when you are at wymthalak, jump down and you could pickpocket the first mobs again. Hearthstone in Ironforge. Against the data of wowhead the droprate is arround 10% (look at wowwiki)! For 65 Boxes i need 35 -50 minutes (with hearthstone Ironforge and back to LBRS).
Look for Heavy Junkboxes in the comments how to quickly pickpocket the mobs.
Tyrs Hand is only nice (not better) if you farming DM with your main and log out actually to get back to the entrance (reason: lbrs cost an instance id, tyrs hand not. You only have 5 ids per hour on your whole server/account).

Insane does not even begin to describe this achievement. Torture is more appropriate. I have yet to see anybody on any server with the title, and I've had toons on three. IF you complete this, you are truly elite.

To much work when the only reward you get is a crummy title. Make it worth more by giving something else like a cool mount or something, Anything.

I lost my sanity aswell.. I do not want to add another comment about pirates :) It's now obvious for everyone that you SHOULD turn in cloths when you are hated with cartel. Make your OWN plan, pop some energy drinks and that achievement is feasible. It's really important to have 2 bank characters to camp both alliance and horde AH. Do not even dare to farm herbs in order to create lesser decks. My suggestion is that earning gold is much easier than farm all those mats you gonna need. (Diamonds are NOT farmable so do not look for the best spot to find them)
Feel free to create a character and ask me if you need any details for your journey!

The above of the most practical guidelines to follow (fairly close with what I ended up doing).
A few additional things to keep in mind are vending and mailbox needs. By now it's worthless (at least on Deathwing) to sell the ogre suits, so between simply wearing the suits and having libram mats on hand eliminates the need to go to town (I vend on a Traveler's mammoth if I did not do a libram turn-in).
Paladin tip:
Go prot, make only 3 pulls per instance - takes you 6-8 minutes to get to Knot. Stop only to loot bodies (loot everything, barring insects or dogs), tannin basket if you are low on tannin, or Knot. Brute like to knock you up so if there are 6+, you can DS to run unhindered. It is possible to do pull 1 &2 together but you'll have at least 10 Brutes on your butt... not particularly fun.
...pull 1: pull whatever's in reach of the straight shot from the entrance to the first boss (stun him), go up the ramp, there should be a little niche straight ahead; park yourself there with your pack once you pull Kreeg. (the mages become annoying as they run out a little ways to cast FB; Arcane Torrent + AS usually pulls them back in). Loot (dump the yellow Inner Key).
...pull 2: Same thing, pull everything in arm's reach in a straight shot towards the center of the next courtyard. If you have been running since the entrance you should be able to grab Fengus along with whatever else. Go down the South ramp (facing the locked courtyard door it'd be to the left), and hide on either side of the ramp. Loot and kill.
... pull 3: Grab key, unlock door. Heal to full, mana up to a decent level (your mileage may vary, my tank gear is only at Naxx25 since it is offspec). Sacred Shield yourself, and start running, grab absolutely everything. Be sure to drop at least 2 Consecrates to kill the bugs (you don't need their loot anyway). Cleanse off Cripples, ignore other stuff; once you get to the top, check DS (very important), pop it and jump all the way off to the bottom ramp, and around the center divider wall (LoS; you can just wait at the first turn I guess). Don't loot if you previously had a key, go free Knot, then check loot. After all that you can check the red book spawn beneath the bottom ramp and grab your tannin before leaving for reset.
Sometimes I get no keys, usually I get at least one (three in one run is the pits ;\). If I run in with a key already I simply skip to pull 3, free Knot, then work backwards if I do not get another key. I skip the very last section half out of laziness, half out of efficiency (too many dogs, nothing necessary drops out of the chest). While I find this is much faster than doing it ret, I frequently run into the hour reset limit, and cannot find a mod that tracks it (Loot & Reset doesn't seem to work atm). Rapidity librams drop most frequently, followed by Protection, then Focus, for me at least.
Best part of DM farming is the Happy Rocks. I save them up and chuck them at people who afk over the Dalaran north Bank well.
Good luck.

Can we please get confirmation on whether you need to keep the reputation at the Achievement requirements to earn the achievement (Mainly talking about Cloth hand-ins for Steemwheedle reputation), for patch 3.3+ onwards.

So as of now in patch 3.3 do you need to have all of the reputations at what the achievements want them at? Or can you just have had the rep there at one point. I'm having trouble deciding what rep to do first if the bloodsail rep needs to stay at honored. Please and thanks for the responses! <33

I am done with everything except Ravenholdt and the Goblins and I'm getting very close to the Goblins. I used a script mentioned in an earlier post to track my achievement as I go, and despite trashing my Bloodsail rep with Cloth turn ins I'm showing the Bloodsail part as completed.
That said, I advise doing the turn ins starting with whatever cloth is cheapest on your server. On mine the order is
linen, silk, runecloth, then mageweave. If you start on Linen and get to nuetral with Ratchet, the splash over rep from doing the next ones up (ie silk in booty bay, runecloth in everlook) will get you at least half way through friendly with Ratchet. After that, I alternately killed Wastewanders in Tanaris and Southsea pirates in The Barrens, depending on my mood.
I don't know why no one has mentioned it, but while wastewanders are further apart then the pirates in Lost Rigger cove, they drop a lot of the water pouches. Water pouches, according to the quest page, are good for rep until exalted, not a huge amount, but enough that it is completely worth killing and looting these mobs as long as they give rep (up until friendly), also, buy any that you happen to see on either auction house, and try offering to buy them from lowbies questing in the area. Also, mageweave is a good way to make back your money from those nasty diamonds, though you'll see more "tough scorpid" leather patterns than you can stand. Also, I've noticed that while you cease getting Gadgetzan rep at friendly for wastewanders, you continue to get 2-3 splash over rep for the other factions and Caliph Scorpidsting awards around 15 rep even at this point. The chests in this area are also great for truesilver bars, a nice addition. I do suggest doing this with a vendor mount to get rid of your trash drops.
Ratchet Pirates are also great for rep simply because of the fact that they can be dispatched with a single "ice lance." I macroed "/tar southsea /cast ice lance" and ran around having a field day. I stopped looting after awhile because if you don't stop to loot you can make a full circle from the bottom, starting right after the area with the theramore marines, up the little path, over the top of the bluff, slow fall down into the first path, and start again. The loop is quick and provides a ton of rep. I'm planning to max out my goblin rep then finish leveling my rogue. I just hate dividing my attention between two toons when there is still so much to do on my main, but it is so close now I can taste it.

Here is my the achievement as i did it.
I started with the Bloodsail rep. I did it with a mage and it only took a few hours.
After this i found a Rogue in trade channel, which i payed 15000g to find me the boxes i needed for Ravenholdt rep.
Shendralar rep: This is the pain in this achievement which i saw many have mentioned. The Pristine black diamonds are rare. Forget what ever ppl tell you or you have read about farming these yourself. During the 2 months i used to get my insane title i only managed to find 1 myself! Buy all you can on AH and also create an alt to check the opposite AH aswell. I bought about 60 of my librams and farmed the last 24 in Dire Maul north. The Blood of heroes, Abomination Stichins and Skin of Shadow are easy to get. Don't (like me) be affraid of buying Libram of Rapidity. The blood of heroes are easy to find.
While i checked AH for diamonds i focused on my cartel rep. It is a lot faster to get rep by killing the pirates south of ratchet than to farm the key quest, but when you reach revered with Ratchet you have to do the "free knot" quest untill you reach Exalted. Don't even consider go to tanaris to kill pirates since the spawn rate and health on them are too high. Ratchet is the place. In DM you are not sure that you will get the key. I was unlucky and tried not to get a single key in 4 full clears!
The Darkmoon Faire rep is expensive. Like other have wrote there is 2 ways. First one is to make rogues deck from low lvl herbs. But you need 1400+ decks to reach Exalted, and the chance you get the hunters ink from milling is so low that you shouldn't even think about this. Go for the epic deck. I farmed herbs in Sholazar Basin. This will take like forever to create the epic decks all by yourself so i also bought a lot. Unlucky for me, the average epic deck price on my server was 600-800g each (Nobles deck not included) which made it even more difficult. You will no matter what need insane amounts of gold to get exalted but you can sell the trinkets with only a little loss on gold. If some shouldn't know, you can turn in 10 ink of the sea for 1 snowfall ink at the Inscribtion trainer in Dalaran.
Hope it could help you a bit to get your imba title!

WELL! i just got mine today... prematurely from what i thought also... i did bloodsail when i was a fresh 80 DK early in Wrath, and after that i did loremaster and got my steamweedle rep up there, started grinding that in DM, got my shen'dralar rep, darkmoon faire rep, and ravenholdt, and then steamweedle to exalted, and no achiev... paged a GM and they said since 3.1 you need all reps simultaneously(sp?) so i started grinding bloodsail back to honored, to redo steamweedle also... as soon as i hit honored bloodsail(hated by all steamweedle) i got my achiev...
this IS in 3.3 here is a link to screenshot

I got about halfway to completing this before coming here to get some advice. Seems I am doing a few of the things mentioned and a few things not. I have found though that the easiest way to go about getting most of this done is to have other people do as much for you as possible saving your time for grinding the BoP items that are necessary.
With the amount of gold floating around in the game today I beleive this Feat has become easier in time consumption but costlier in the amount of gold needed to complete, at least the way I am going about it. I find my guildees are usually in need of gold for one thing or another so I gave then pre-set prices for what I need. They CoD me, gining them the gold they need and the mats I want.
I use five level 80 characters and one bank alt to support an income of about 5000 G each week and all the professions associated. All this gold goes back into the AH or to guildees for Librams, Pristine Black Diamonds, Heavy Junkboxes, and the herbs needed for DMF cards. I hang out in Dungeon Que while killing pirates for the Cartel rep. Looting the pirated takes a bit of time but the money I have made selling bolts of cloth has gone right back into getting the mats for the other reps. All trash goes to the mammoth vendor saving me time to continue killing pirates.
Once my bank alt has sufficent mats pre package them and head off to DM to start turning in Librams my Argent Squire has readily available. I usually do about 10 or so turn-ins before the squire will not let me have access to the mail box. Logout and come back in he morning ready to turn in some more.
There is no way around getting the BoP mats and I am not doing anything other than what is already atated here.
Hope this helps someone. It is not the fastest or perhaps the easiest but it give you alot of time off the grind while playing and gearing other characters.

I have done the Steamwheedle grind both ways and have to say that in hindsight doing Dire Maul was much easier and more profitable. I ended up selling the epic patterns I'd get from Knot's cache and actually made several thousand gold selling them on the auction house. I also got a bunch of Darkmoon cards as drops in DM, a few Pristine Black Diamonds, about 60 librams (the rest I had to buy on the auction house), tons of runecloth, old school recipes, all things that I was able to sell for a profit. The trick with DM is to try to go in with a key and come back out with a key, and generally a key does drop (although, as people have noted you will get unlucky a few times and not get a key at all!). Also, do the Ogre suit quest, as the cache usually contains rugged leather you can use and just restock when you open the next cache (I'm a tailor so I was able to make the bolts of runecloth, but you can always have a friend or guildmate help you here and make a bunch of bolts for you). I noticed that if you are, say in 251 gear when you grind pirates you are going to be losing money because you get a repair bill when you kill them for long periods of time without stopping. You also won't be getting much money (if you loot them at all) or be working simultaneously on Shendra'lar rep. And, I have to say, I literally went insane from farming all those pirates for so little rep apiece the first time around!

Finished this entire Achievement over the course of 1 month. I play a level 80 DK, had a level 70 rogue alt.
Farmed all Librams in DM West, at the same time partying with my Rogue alt who had enchanting (to get LBS).
Got all 84 in 4 days. The next 3 days I did all the Skins of shadow, Blood of heroes, Abom stitching (This whole time buying PBD's from the AH). Was exalted with shen'dralah after 7 days.
I then went and did Ravenhodlt, took me half a day to get to revered, and then the next 4 days I got all the lock boxes, at about 300-400 a day.
I did 2 hours of quick farming for Honored Pirates, and then started on Goblins. I've only done key runs, and maybe the time has taken me longer than it should be it's taken me 8 days of farming to get to exalted.
This entire time I was buying Greater decks from the AH skipping nobles for no more than 500 gold. This part of the achievement took me the longest, but really buying from the AH isn't hard work at all.
If you add these days up you get 19 days. That's basically 3 weeks of farming for the insane title for me. Sure I spent heaps of gold, but this achievement really isn't as hard as people say it is.
I made this post not to tell people strategy in how to do it, but show people that it's really not a HUGE effort that people make it out to be.

I am in the process of going for this achievement, do not attempt it if you are in doubt about following it through as you will lose all your goblin rep if you have done the quests which means getting it back will be a chore.
As it stands I have just got to hostile with goblins after many, many runs of dire maul, 5 more librams though and I have my exhalted with shen'dralar. My advice for these is get as many librams of protection as you can 1st. The drop rate for frayed stitching is a least 1 per strath run and you only need one for the hand in, unlike the focus and rapidity librams which require 2 of each correpsonding mat. Shadow skin as a fairly decent drop rate but not as high as stitching. Blood of heroes is seemingly very random. I went 3 days with out a single one then 12 turned up.
Its worth doing the ogre suit quest, 75 rep may not seem much but your looking at approximately 200 dire maul runs if you want to get rep that way based upon just the key quest. Many people just kill the pirates after completeing Shen'dralar. I'll try this out but as an enchanter I find that I am making money out of de'ing the loot from dire maul, strath and scholo (thank you blizzard for boa gear, I sell the mats then charge fee for enchant ) as well as getting bountiful supplies of runecloth for the ah. Certainly helps pay for any librams and diamonds you may want to buy to get it done quicker.
Darkmoon is going to be expensive, but be clever, it comes round every few weeks, use that time to buy individula cards, on my server I'm paying anything between 15-110 gold for a card but full decks are average of 500. If I pay 110 for a card its usually the final one I need to complete the deck so it does work out cheaper. Some people offer money to hand in decks for others before passing the trinket back, this will work okay unless some twat ninja'd lots of people doing it in which case the uptake on what is a great offer will be low. Obvioulsy it depends on the economy on your server.
Ravenholdt, not sure on this, havn't done much yet apart from kill a few mobs to satisfy curiosity, seems like a nice quick repsawn on them and very easy kills. You can either just kill, kill and kill or stop to loot, depending on your server is what makes the difference. stacks of woolcloth sell for 10-15 gold on mine on a good day so its worth looting them, if they sell for pennies then ignore. I have a rogue alt so lbrs farm will be boring but cheap. if you have lots of gold or no rogue then pay someone else to do it in my opinion.
Anyway, if you are undertaking this then I wish you good luck

This is to clear the mysticism surrounding the instance cap that people seem to be confused about and provide a means to keep track of it if you want to seriously min/max your farming efforts. The instance cap is set at 5per hour, meaning you can only hold onto 5 unique instances in a one hour chunk of time.
Once I've finished my first run and have reset the instance and zoned into the second run I check the time and jot it down. One hour from that time I'll be able to zone into my 6th instance. To push the envelope you want to jot down the times for all of your runs, so that you can better keep track of when your consecutive instances open up. For example: If I started my 2nd run @ 1405, started my 3rd run @ 1415, 4th run @ 1430, and 5th run @ 1438 it's pretty easy to see that you would have to wait another 27 minutes to zone into your 6th run @1505. To start your 7th run you would have to wait until 1515, thats when your 2nd run would fall off, and so on.
For people who are seriously considering this feat it is highly advised that you have an herbalist & inscriptionist alt. My server seems to have a severe shortfall of inscriptionists who make cards so finding decks/cards to buy is a harrowing experience. I find it necessary to supplement this by making my own lvl70 decks. I keep a lvl80 herbalist ret paladin in Skettis and while I'm on wow, I'll do my best to log her and kill the Talonsworn Forest Ragers who seem to respawn every ~5-7 minutes. Each mob can be herbed for 2 random drops of 2-5 outland herbs and 2-5 motes of life. There are a total of 4 spawns. After a lot of playtesting this seems to be most efficient way possible of creating cards for "The Insane."
In my experience the best places to "farm" pristine black diamonds would be Scholomance and ZG. If your AH never has PBDs on it or if they are always posted for 400g+ and you're a cheap person like myself I would suggest trying these places out. ZG is the most cost benefit for your time due to the chance of selling bijous/coins and possibly the raptor pets but Scholo is nice as well while you're grinding out those skins of shadow or dualbox grinding your alt rogue levels.
Godspeed.

Short background, I am working on Insane in the membrane title and the last two parts i have to do are Shendrelar and Darkmoon. I have all the stuff i need for darkmoon and am just waiting for the date to turnin. For shendrelar i got 23 focus, 39 rapidity and 15 rapidity. I wish i had gotten more rapidity but so it goes... So after reading this info on the gathering of these I was almost dreading the 78 blood of heroes I had to collect. I kept putting it off. The other day i had a couple days off work so I decided to get started. The first attempt at it I only managed to get about 6 in a two hour period. I'm not sure if somebody else was out there or what. The second night I started around midnight. t his time i used a friends account to duel box with a paladin for the crusaders aura. I had used the wowdatabase and gatherer to show all the point s on the map. First time out I got about 30 of them in 3 hours. I did this by starting up around strat in EP then across the crack around the middle of the map over to the lights chapel then down around tyrs hand. I basicly played connect the dots for a few hours. The were just flowing one by one. it was amazing. I decided to sleep and when I woke up I got back to it. I again followed the same course I did the day before, this time I also moved over to WPL to try to get move. They were even easier to find than before. I collected a total of 78 in about an 8 hour timeframe. I wish i had more of them to collect at this point because i think its easier than the scholo and strat grinds for the other parts. Anyway, April 4th i will be Krazycalvin The Insane!

Two points on this.
Firstly, Dresorull confirmed that they changed the rep requirement to "At some point" instead of "at the same time" in 3.1.
Secondly, to make the aftermath easier, grind your Buccaneer rep to Honoured first. Start by grinding up 168 stacks of Runecloth, 84 stacks of Silk and 42 stacks of Linen or Mageweave. I did this in Stratholme and Scholomance while collecting the stitchings and skins.
Then burn up your BB rep while getting to Buccaneer Honoured, and once you're done with that step, go and hand in all the cloth, Everlook first, then Booty Bay, then whichever other one you chose. This gives you free rein to travel again and use things like the Neutral AH. It also means the Free Knot quests don't need to get you as much rep as they did.
You might call it 'micro managing' but I call it being able to pace out your rep grind while still having access to the BB<->Ratchet boat, and the various flight paths etc. You don't have to do Free Knot repeatedly without being able to use the boat. It also means not having to fix up your Steamwheedle rep afterwards, and since we're all completionists, you finish with 4 exalted reps instead of 1 honoured and 4 hated.

Just for clarity, Insane in the Membrane will not be removed in Cataclysm. We can only confirm this one for you, but since it's undoubtedly one of the most brain-melting achievements, we thought it nice to let you know that you don't have to rush to have this done before the expansion. ;)

So, I've seen a lot of posts about different strats to getting this achievement/title.
But everyone seems to be forgetting one HUGE thing.
Make sure you're keyed for Scholomance before trashing your goblin reps. I personally didn't think about that, and was forced to grind the goblin reps back up to neutrl before I could think about touching Shendralar.
Here's my strat.
Prepping-
1. Get keyed for Scholo, if you're not already(or if you're not a Blacksmith/rogue)
2. Level a rogue. Most people say you should have a rogue thats lvl 65+. If you don't already have one, there is really no need to go above 55. Just make sure you have the appropriate points in subtlety.
3. Bank alt. Most of us have one anyway. Yours will be a nice place to store Librams, or possibly junkboxes, until you're ready to use them. Also can be used to search the Auction House for Pristine Black Diamonds and cheap Decks/Cards.
4. Farm the BoP items for the librams beforehand. Not all of them, just enough so that when you go to Dire Maul, you don't need to use a mailbox/squire very often.
Grinding-
1. Pirates- Destroy Booty Bay for around 2-3 hours. If possible get a few people to help. (I got lucky and found a few people that wanted Bloodsail Admiral, and as soon as they found out I was going to work on this achieve, were happy to help with the extras.)
2. Goblins- Cloth turn-ins until neutral. Yes, this trashes your bloodsail rep, but as stated several times before, you do not need all the reps at the same time.
3. Shendralar- Run Dire Maul North. Stick with North unless you're already exalted, or have been exalted with the goblins. The West wing has a better drop rate for the librams for a lot of people, but the keys will be needed to turn get exalted with the goblins.
4. Ravenholdt- Farm syndicate mobs in Durnholde Keep in Hillsbrad Foothills until 11999/12000 Honored. Then it's off to LBRS. Don't waste your time at Tyr's Hand. The droprate of the boxes is so low there, that you'll burn yourself out before you get started. If you have a large stock of gold, you may also buy junkboxes off of some friendly rogues on your server.
5. Darkmoon Faire- Rogues decks work, but very slowly. I'd only suggest using Rogues if you are a very small distance away from exalted and the Faire isn't active. The best way to do this is to have an Scribe yourself, or possibly a guildy/friend that makes a lot of decks. I found one that made 20 cards per day, and sold the rep from the decks for 200g (150g for guildies) each.
The biggest thing about this grind for me was to avoid getting burned out on one single part. If you feel yourself getting bored or frustrated with one part, move on to another. And whatever you do
Ignore the people that say you'll never make it!
Happy Hunting

Let me begin by saying... you guys have it so easy now. I even had it easy, just cost me a lot more cos Darkmoon Cards were expensive at the time. But i still agree with the first post when he says if your in for an easy ride then forget it! Took me 6 months of making cards through inscription and about 2 months of full on pirate and mats farming.
First things first. Decide your route. Some people dont have certain professions that can benefit your journey. For example Inscription. I decided to level it on my main for the extra shoulder enchant anyway, its a great prof. If you cant or cbf lvling another prof for this achieve then my suggestion is to just buy decks/cards.
If I miss any factions, its because they are easy and you can figure them out yourselves =D
Darkmoon Faire...
As I said, i decided to lvl inscription for this, for a few reasons. The guide says to buyout rogue decks... that is stupid and a waste of money. I worked out that it costs aprox 43k gold... too much... so I made the decks myself. I made all my decks, starting with 2k gold. When id finally reached exhalted I had made back about 45k gold. The price of herbs now is a joke. So the profit isnt as great, but the cost is so cheap you can get to exhalted in no time.
Auction House for cheap decks/cards is another great option. Some people just have too many cards and sell them for really cheap. Excellent for cheap rep.
Another thing which hastened my journey was having a few people to trade nobles or other cards with. This really helped. Maybe a guildy or friend also farming The Insane.
(At this point, if you have the spare gold I highly recommend a Tundra Mammoth. the vendor/reagent and the repair is priceless when farming mobs)
Shen'Dralar.....
This one gave me the poops.... And so did the many guides i read about it.... First things first.... collect the friggen side mats you need! Blood Of The Heroes mainly because the spawn rate. They are the rare spawns of blood which appear randomly in EPL and WPL. While you farm these its also good to get stitching and shadows from strath/scholo and buying Librams and Prismatic Black Diamonds whenever you're at the AH or in trade.
For the Bloods/shadows/stitching, i would do 2 whole runs of the whole area of EPL/WPL and collect what bloods I could find. After that I would spend 2 or so hours in Strath/Scholo. Every time I reset I would check the area Bloods would spawn close to the dungeons and repeat. This took me a long time.... But its a lot about luck. I had a friend who got them all in a few hours.... curse his luck! It took me about 60 hours over 5 days...... long days....
AFTER you have a mix of 86 set of side mats (If your an enchanter, dont bother buying Large brilliant shards. DE all the bosses in DM), This is when you enter DM and farm librams, If you have already bought some librams and Diamonds then bonus, go hand in what ever you can. At this stage if you have a friend with Engineering who can make you a mailbox next to the guy u hand in, double bonus! I had a lvl 60 priest mate going for the insane with a warlock helping me. We had his warlock summon us hehe. took ages, but its the Insane so yeh...
(At this point you want your goblin factions at revered)
Now you'll be farming bosses and ogres for librams and any keys you can find. I like to do a bit of math for running dungeons, and you'll have to run DM several times before finishing this achieve. If you get locked out thats a lot of down time, which is the last thing someone farming this achievement needs. 5 to 6 runs in 60 minutes is what you want. so theres 2 ways you would want to do this. 5 12 minute runs, or 6 10 min runs. obviously. Thus, preventing being locked out =D
Now keys and librams need to be taken into account when doing runs. They are unique and so you can only hold 1 of each(1 of ea libram). Lets assume you have no librams and no keys, you would farm all trash and bosses. If you have 1 key and 2 librams you would farm only bosses and trash that needs to be killed. Most trash you can avoid at 80. Personally, i would get 2 librams then hand in, always having the mats i had previously farmed on me at all times.
Another thing to do each run is to farm Ogre Tannin. That gives 75 shen dralar rep per hand in and chances to get up to 2 more in the ogre tannin box when you hand in. where you get the ogre tannin is where i would have my end point. I didn't bother going to the last boss. Also I dont think he drops a key.
Goblin factions + Bloodsail...
Obviously do Bloodsail first. Some keep farming rep then handing in runecloth to increase goblin and lose some bloodsail. Thats all well and good, but in the end of the day your going to have to farm some pirates.
FORGET ANY GUIDES that say Farming keys to do this is faster.... its not, its a waste of time at the early stage! After trying every spot mentioned in any guides, I concluded that the Pirates in Tanaris. Just behind Caverns Of Time. It takes around 17,000 kills to get to revered with all factions. Most people just stop once they reach revered with Gadgetzan.... You need to realize something. Goblin Rep is all about Everlook. There is no place to farm 5 Everlook rep. only 2 or 3. So just stick it out in Tanaris until your revered. Get some music ready =D a lot of it.
Once you reach revered is when you start farming DM and get both Shen'dralar and Goblin rep at the same time.
Ravenholdt all depends on your access to a rogue. 3 options. level a rogue, use a friends rogue, pay rogues to help you farm them. I chose options 2 and 3. I used a friends rogue and paid ppl 5g per box to farm in LBS.
The little advice ill give, because personally this one was the easiest for me, having a mate that let me use his rogue, first glyph with pickpocket range. 3 extra yards is excellent for not getting caught. Second, make a macro;
/targetenemy
/cast pickpocket (correct me if im wrong and ill edit. it might be /use not /cast)
I had that setup on my bars and just ran around spamming it. took me no time at all.
I hope this helps in one way or another =D Enjoy the farm!

Just to add on to all the comments about farming ratchet rep off the pirates south of Ratchet the equation (and yes I did this and died a little inside) for the rep is:
(amount of rep needed / 5 + amount of rep needed / 6) / 2 = the total number of kills necessary to get (lets just say) to neutral
Although Baron Longshore counts as 5 kills by himself.

Ok strategy I guess, but nothing thats reportedly faster.
Here's why:
* When farming Gobling rep you will do about 220-250 runs, or more depending on how many shackle keys you get. During that time you can also farm a *ton* of Ogre's for Ogres for Shackle keys (1 pr run, and 1 for the way out), Libram's and Pristine Black Diamonds. Librams dont drop more often than that when you have filled 100+ bagslots with random loot, you're likely to have found the librams you need.
You need 89 librams total for shen'dralar, and you're not going to get 89 pristine black diamonds by farmin Dire maul. Most people see 4 or 5 drop in the whole Insane grind, and if they got more they're either lying or got really lucky, getting more is not the norm by any means.
You should create a alt guild bank to store Librams that you either farmed or bought of trade chat/ah as they can store as many as you'd like. Dont waste time running to turn in 1 and 1 libram during your Dire Maul farm as its just a waste of time if you can avoid it. (Doesnt apply if you cant get rid of the one you're already carrying and one more drops, but hey).
Remember, if you get a drop, you always have your Argent Gruntling ready to send some mail away and make room for more!
A important sidenote: IF you choose to grind pirate rep, after you did Dire Maul, you're going to get to Hated with the Goblins, effectively making you unable to enter Rathchet, Gadgetzan, Booty Bay etc without getting attacked on sight. If you want to be able to visit those cities, you do the Pirate grind FIRST!

This is easy, but very time consuming. A few things you should start with to make it easier are obviously they are all optional but the one that say's optional before it i just mean that it helps but are in no way a major needed helper
1. a lvl 80 with a good source of income (someone who either has a lot of money or can rake it in by the thousands)
2. (optional) a group of people also after a rep or the achievement itself
3. hopefully your character is 70+ because the higher lvls will make this so much easier
Bloodsail reputation: (I have completed this part so i am knowledgeable about it) First things first go to your rep tab and set yourself to at war with steamwheedle cartel. First this is
VERY IMPORTANT
there are TONS of lvl 80's who if you should go to stormwind and say "LFM for Bloodsail Admiral achieve" you will have several messages probably. After you do this start somewhere in booty bay (i start at the top where the leaders are) and work your way outside. Once you do this you will probably be about halfway to neutral. repeat until you are able to get the quests from the bloodsail buccaneers. this will only take a few hours varying depending on level.
I cant help with any of the other parts yet but i will update soon (i am working on this achievement all day tomorrow and will probably update tomorrow)

As long as you meet the requirement once, you're fine. In other words, upon reaching honored with bloodsail it forever gives you credit for that fact. The achievement does not say get these reps "all at the same time" or anything of the sort.
Hope that helps :)

OK guys, here's the answer to the question so many people ask. You DO NOT have to be Exalted with Steamweedle and Honored with Buccaneers at the same time to achieve Insane In The Membrane. I became exalted with the cartel FIRST and was very worried that I would never be able to gain the rep back once it reverted to hated as I fought on the pirate side.
SO ONCE AGAIN, IT DOES NOT MATTER WHAT ORDER YOU DO THESE IN!

do you kill the mobs after you pickpocket? or does the pocketloot respawn itself ?
well that was rough but i got 'er done :D thankfuly i'm an engineer and could pick about 80boxes to fill up my bags then pop the mailbox CD and continue on.
Did it all in LBRS, was a good drop rate.

Question -
HOW certain are we that you do no need to be Honoured with Bloodsail and Exalted with the Goblins at the same time?
I'm Honoured with Bloodsail at the moment, just maxed out Honoured with Ratchet with pirate-farming, but decided to check out this page for some tips. And then i hear it's no longer an 'at once' achievement?
Of course when I check the achie status with the link function, everything's greyed out so I don't know what's been 'counted'.

Just something I thought of and thought of being worth for sharing.
Somehow I noticed that while being in a group u can gain reputation with the bloodsail faster while nuking goblins at Bb, in difference to all the mobs we must kill to achiev the insane with goblin exalted later. Now I am suggesting, when u hit honored u will probably be 0/36000 with the goblins at hated. But go on killing dudes and get 11999/12000 of honored! Why? Cause u can easily do
such qs
quiet sometimes after. Like that you can get 11k of goblin rep back faster =]

Here's the proof that you dont have to be exalted with Steamweedle and honored with the Bloodsail Buccaneers at the same time:

You seem awful %^&*y with your strategy and I see plenty of wasted time involved, including grinding out all of the mats for the librams before you even have them... doing this assumes that you will get all of the right librams to drop, and there is a possibility that the mats that you spent hours farming will be wasted if you end up not needing to spend them. Everything else looks pretty good to me for the most part.
I have been planning for this achievement for a while and plan on hitting it full force as soon as I am done with loremaster. And this is how I plan on doing it.
Bloodsail- Of course just grind Booty Bay guards, I see no way around that.
Shen'dralar (or however it is spelled)- I think people will have to work indirectly on this rep for a loooong time, considering the drop rate for pristine black diamond is so low, and you will need about 77 of them(im a human). I suggest checking the AH every chance you get and buying out every one you see that is under 75g, and advertising trade once in a while on the off chance that someone has a couple sitting in their bank that they would get rid of for cheap. (this has happened a couple times for me(JACKPOT)).
The librams... I think that people are making a bigger deal of these than needs to be made. hear me out... Just advertise trade for a bunch of level 45-60 lowbies to get free runs through dire maul(most of the time you will get a couple takers) after you get them there you just go hog wild killing everything in your path, and having the lowbies loot the bodies. Since there will be more than one person... assuming you have 5 people in the party, that will be 15 librams before you have to drop a mailbox(if everything goes perfect) and even if it doesnt go perfect, you should be able to carry all of them until your mailbox is cooled down again( for gods sake either be an engineer, or do your argent tournament dailies to get the squire.) if you dont have one of those... i cant begin to fathom how much time you will be wasting.
Every time you drop a mailbox have your party members send them to a designated char(one of your alts) that has their own guild bank(i happen to have a toon that is the only one in his guild with a 3 tab guild bank) If you dont have an alt like this... make one, it is easy and cheap to get a guild bank, even if it is only one tab. At the end of the day I just log onto my Gbank alt and put all of the librams in the guild bank (you can put as many as you want in there, even if they are unique), once you have the required amount of librams, you can start grinding the mats for them, and you will know how many of each you need.
The goblin reps- Do the repeatable quest inside of dire maul every time you run your lowbies through. If you get excruciatingly bored of this, I think there is a repeatable quest on the coast of feralas that just requires you kill water elementals that are plentiful on the coast. I dont see this being one of the more difficult ones in that you dont have to do lots of different things, just hours of mindless grinding.
ravenhold- like the guy above said, get your rogue buddies to help you, or hire them. if you have a rogue make sure they have increased stealth speed, and as many weirdo speed increasing things as possible... goblin stop watch, swiftness potion, goblin rocket boots, movement speed enchant, you know. again, just hours of mindless grinding.
Darkmoon fair- The way I am approaching this is checking the AH every chance i get. just click the "quest" tab, and type in "of" in the search bar, and list them by the lowest bid. I go through and buy out every blue quality card i see that is under 15g(there is usually 2-3 of them that are only like 4g that people put up there not knowing what to do with them) like the librams, send them all to your Gbank alt, and organize them in the gbank so you can easily see when you almost have a complete deck. check for cheap decks also... but chances are if someone knew how to put a deck together, they knew how much it was worth, i have not seen an epic deck on the AH for less than 500g, and if you follow my above plan you should be able to get a deck for only 120g MAX.
Hopefully I gave some helpful suggestions to take the insanity requirement down for this title.

I got this achievement a few weeks ago, the farming part is not that hard, if you have the time and you usually spend it jumping around in Dalaran/Orgrimmar/Ironforge, and you want something to do, I recommend this. Took me around 3 or 4 months for my title, the last month I spent scanning the AH for decks. During those 3 months I might have spent 1-2 hours farming every day (sometimes more, sometimes nothing).
I have a very good tip: During Darkmoon Faire you might want to advertise that you want to trade in other's decks for them, just for the rep, and you want to pay them. I did this with my last ~30 decks, for every epic deck they wanted to turn in, I paid them 500g each, and gave them the trinket. On my realm this is a real bargain, since there are never more than 3-5 epic decks on the auction house, and they sell for 800-1500g each, and the trinkets re-sell for less than 500g. Berserker!'s sell for 150.
Want to recommend this for anyone in the late stages of the achievement, at this point you will know how hard it is to get hold of them on your realm :)
Good luck!

While doing this achievement, you may want to check all the
Heavy Junkbox
you get for the ultra-elusive
Teebu's Blazing Longsword
, but be sure to keep something else in the Junk box otherwise it will disappear. Also, if you manage to complete the
Shen'Drelar
reputation before the
Steamwheedle Cartel
reputation, you may want to farm the
Bloodsail Buccaneers
for the lowest drop rate item in the game, the
Hyacinth Macaw
while doing the last piece of the rep (the longsword may have a lower drop rate than the pet, but it cannot be confirmed because it has only been recorded to drop 4 times, so the odds may as well go up to 1/100,000). These items will probably sell for about up to 50,000 Gold, which is enough to pay for anything in the game. Also, if you accidentally didn't do the scholomance key questline you may as well go ahead and farm the hyacinth. Also, if you level a rouge for
Ravenholdt
, try grinding with the pirates to increase the chance of getting a Hyacinth.

I said
may
want to
if
they wanted some kind of elusive reward/had the right rep or, taking in to account that
if you complete it and just get all items that are green and higher and sell it it would be more than the Hyacinth macaw
would you not double your profits? This is just a suggestion, because it would save time if you were to want to get the pet in the future - you would have two farms in one. You may also be able to buy a lot more things with the extra money, instead of spending that on the pet if that were your intentions.
Also, other hard to get items/achievements like the
Time-lost Proto Drake
has many long and descriptive comments.

Tip on the Ravenholdt part: If you're enchanter, it's is TOTALLY worth it to disenchant every single Dagger you get from the Quest. I'm still on this, as my Rogue friend is still farming these boxxes for me and after 7.5/21k, I've been selling all of these Lesser Eternal Essence for 5.5g each, and dream dust for 40s each. Though, before making Greater Eternal Essence out of this, you'd better check the AH. Lesser ones turned out to more expensive then Greater ones on my server, and as it costs nothing to put Enchanting materials on the AH, it's no problem if you'll have to try and sell it a 3rd or 4th time.
With some quick math, you can see that you'll actually be able to earn back quite a bit of the 10k you had to spend on this. Let's take my repstatus as an example:
7.5k rep = 100 Quests completed = 500 boxxes turned in = 500 Daggers.
The daggers drop 1-2 Lesser Eternal Essence 76% of the time, and 2-5 Dream Dust 21% of the time. This results in:
500 daggers = 380x 1-2 Lesser Eternal Essence = ~570 of them.
+ 105x 2-5 Dream Dust = 368 of them.
On my server, this would result into 3135g from Essences and 147g 20s from Dust, making a total of
3282g 20s
. Now this proves to be quite accurate in my case, as I've earned about ~3150g from this so far.
Again, note that this is while I am
at a third of the repgrind
, so I can safely say I'll be able
to earn back atleast 9k in the end.
Prices will ofcourse depend on your server, but wether it's a tad more or less then the prices on my server, you'll probably be able to get back a pretty decent ammount of the gold you had to spend on this.

This is a good way, but there is another way to get bootybay, ratchet, gad, and everlook up without lowering the Bloodsail rep. Its time consuming, but easy. Killing the venture co workers north of booty bay yields 5 bootybay rep and 2 of everything else.

BEWARE OF THIS ACHI IN WOTLK CLASSIC!
Im not sure if its intended or a bug, but even tho wotlk classic is based on 3.3.5 you actually need to have bloodsail buccaneers and steamwheedle cartel reputations at the same time. i did it the wrong way, and now i just wasted alot of hours for nothing and have to do goblin reputations again. as you can see from the comments here and everywhere else, you didnt need to have goblins and pirates simultaneously in wotlk, atleast not after patch 3.1. i just feel so scammed, how is anyone supposed to know this? its literally impossible to know unless someone tries it before, classic is based on 3.3.5 and i even had the checkmark on the achievement.

Don't be like me. Comments here seem to indicate at some point in the past you could achieve Bloodsail/Steemwheedle separately. That is not the case per WOW help article which I only found after not earning the achievement:
"The Wrath of the Lich King: Classic version of this achievement requires a character to reach a state where all the criteria in the achievement are true at the same time. This means characters will need to be exalted with all the goblin reputations at the same time as being honored with Bloodsail Buccaneers. The WoW team has confirmed this is true to how the achievement originally worked when the achievement system first came out before it was later changed to function like how it currently does in Modern WoW."
RIP my time, but I'll get it eventually!

Darkmoon Faire (Darkmoon Faire rep)
Because this is the longest rep grind, it is advised to start it first. Level an inscriptionist. If you don't have a rogue and you need an alt for this, it wouldn't hurt to level a rogue alt up now too. Watch the darkmoon card prices along with herb prices. Do the math. It takes ~4200 herbs per hero deck. For me, goldclover and deadnettle run around 25-30s/ea and decks sell for 4-5k reliably, which is almost doubling my money for each card.
Destroy Booty Bay (Bloodsail Buccaneers rep)
Declare war on Booty Bay. Run around Booty Bay and kill all guards you find. There are good spots to stand in to AoE away from regular NPCs. One end of the dock, one up the dock, one is down the ramp (you can even duck into the right hand building to LOS them all tighter), and one is the big pole near the Alliance FP. If you go to an area and just stand there the guards will stream in for a while then stop spawning, at that point move elsewhere. Get honored with Bloodsail Buccaneers. It would be ideal to max this out and use the cloth turn ins to raise steemwheedle back up until you are barely honored. Keep in mind the cloth turn in will give you a hit to Bloodsail.
Farm the Plaguelands
You will need about 10 each of Shadow Skin and Frayed Abomination Stitching, plus a LOT of Blood of Heroes.
Shadow Skin - farm Scholomance, sneak in, over the bridge, turn left, then go into the room and on the left side are holes to drop down. Kill the boss in the bone room, then go back through and you will be in a room of mobs immune to boomkin spells, you should be able to go through the other door to the final boss. Kill all 6 mini bosses and the final boss. Some drop skins. You should average 2/run.
Frayed Abomination Stitching - run Stratholm dead side, basically just go kill Baron Rivendare. The stitching is on the abominations right outside his chamber, so you are inches away from an attempt at the mount anyway.
Blood of Heroes - Get Questie, search for Blood of Heroes, select "show on map", run through each of the nodes in Western Plaguelands. Once you loot one, at least one will spawn elsewhere. You will need a lot of these, probably 30 to start with.
Farm Dire Maul North (Steamwheedle Cartel and Shen'dralar rep)
Buy a stack of Pristine Black Diamonds and 2 stacks of Large Brilliant Shard too. You will get some diamonds farming instances. Also buy a few stacks of rugged leather and bolt of runecloth.
First do the Elven Legends quest. You also need access to DM north if you do not have the crescent key yet. There are a couple other dungeon quests like Pusillin that give Shen'dralar rep.
You will farm Steemwheedle Cartel and Shen'dralar at the same time here. Kill ogres until you get a shackle key. Basically the path of going west to the courtyard, get the key in the middle, then go inside and follow the tunnel. You will be at a goblin named "Knot" standing on the right hand side. Talk to him and tell you you have a shackle. He will then have a quest to free him. Off he goes. LOOT THE BOX HE LEAVES BEHIND. You will hopefully get ogre tannins. Now go up the ramp and there is a basket in the corner with another tannin, loot this box. A bushwhacker comes out. Kill him, he has the highest chance of dropping a shackle key. Continue killing your way back until you find another shackle key then exit the instance. At this point, if you have a shackle key, rush to Knot, if you don't, just kill every ogre you see while you work your way back and forth. It is unique so priority is using it before you kill another ogre. It is also a key so it will show up in the keychain, not inventory. I made a weak aura to show text when item count == 1. The tannins you can turn into Knot along with rugged leather and bolts of runecloth to make ogre suits (also unique, so just destroy it) for 75 rep per turnin. Freeing knot is 350 rep.
Along the way you will get one of three books. Libram of Rapidity, Libram of Focus, and Libram of Protection. The second you get a book, go back to the first room of the instance. There is a tunnel that has a door. Follow this unnecessarily long tunnel until you get to a room. Go straight and the first elf on the left is your turn in guy. Each libram is 500 rep. You need 84 turn ins for exalted. You can sometimes buy books (especially rapidity which uses blood of heroes) on the AH decently cheap.
You will now get in a cadence of grabbing books or shackles and turning them in then continuing through the instance. It seems to be a good balance that you should have Shen'dralar neutral->exalted about when you get goblin reps hated->exalted.
Go back and farm or buy materials as you run low.
Level a Rogue (Ravenholdt rep)
First kill Syndicate mobs until 11999/12000 Honored in Alterac/Arathi/Hillsbrad. Get a rogue alt up to 70 or 80 or so, enough to easily pickpocket classic dungeons. Pickpocket heavy junkboxes from mobs in UBRS and send them to your main to turn into Ravenholdt. You will need 1400 heavy junkboxes total.
Congrats. you are now insane. Enjoy the worst title in the game.

FOR ALL OF YOU WHO IS DOUBTING, READ!
short: just got my title, and i have all goblins hating me now! YOU DONT NEED TO HAVE THEM ALL THE SAME TIME (but...)
long: first i was grinding 2/4 goblins rep at barrens just to get 40 exalted reps for achievement (got exalted at WotlK, not earlier, maybe this is important), then i've learnt about The Insane title, read a lot of controversial comments about bloodsail and goblins reputations, but since i had 2/4 goblins exalted already i decided just to test it in case it works and i dont need to have them the same time, so i farmed mobs till revered Ravenholdt, then hired rogue to farm me boxes (as i didnt want to play rogue), and got it exalted, then finished all goblins rep at Tanaris, farmed and bought librams for Shendralar, killed guards at Booty Bay (it was stressful because i realized that i dont just go to neutral 0/3000 with goblins, but hated 0/30k and 10k/30k, predicting how much time it will take to farm it back, and also nervously laughing about the decreasing speed of reputation's compared to hours of grinding 2-3 rep per kill), and today after non-stop headache with all these cards (i'm playing at Nehtergarde Keep, very dead server) i've finally got my title
so, again, i'm not sure (!) it gonna work if you got exalted with goblins before WotlK release
wish you luck with getting this title (but seriously, better dont start if your server is as dead as mine, I regretted three hundred times that i've started it)

A bug ticket confirmed that Bloodsail needs to be maintained.
So do your grinds accordingly.
